About this file

This is a Past Performance Questionnaire template issued by the Air Force Civil Engineering Center Installations Directorate (AFCEC/CI) under Air Force Materiel Command (AFMC) for solicitation FA8903-27-R-0014, dated 21 October 2026. The questionnaire is designed to collect performance information from past customers and contracting officers who have worked with offerors bidding on Air Force Privatization Management Programs (PMP) and Housing, Enhanced Use Lease, Lodging, and Utility (HELU) Privatization Management contracts. These are long-term, cradle-to-grave Advisory & Assistance Services (A&AS) contracts for Asset, Acquisition, and Portfolio Management services supporting privatized Air Force housing and utility programs throughout the United States. Completed questionnaires must be submitted within 10 calendar days of receipt to Mark Ingle (mark.ingle.2@us.af.mil) and Michael I Grasso (michael.grasso.2@us.af.mil), the Contracting Officer.

The questionnaire requires respondents to rate contractor performance across multiple categories using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Evaluation categories include technical performance on housing, Enhanced Use Lease, and utility requirements; quality of technical data and reports; program management effectiveness; transition and phase-in capabilities; employee retention and attraction; small business participation compliance; and cost performance. Respondents must provide supporting information for each rating and identify their role, tenure on the contract, and position (e.g., Program Manager, Contracting Officer). The questionnaire also requests identification of the past contract, including contractor name and CAGE code, contract number, contract type, program title, period of performance, dollar value, description of work performed, and customer or agency information. A final section requests the respondent's overall assessment of whether they would award to the contractor again and solicits additional information relevant to government evaluation.

FA8903-27-R-0014 21 Oct 2026 Attachment L-5 For Official Use Only When completed, this document is Source Selection Information IAW FAR 2.101 and 3.104.

Section L: Present/Past Performance Questionnaire The Air Force Civil Engineering Center Installations Directorate (AFCEC/CI) of the Air Force Material Command (AFMC) is in the process of competitively selecting a source(s) for Air Force (AF) Privatization Management Programs (PMP) and Housing, Enhanced Use Lease, Lodging and Utility (HELU) Privatization Management for long-term cradle to grave management of Advisory & Assistance Services (A&AS) in support of the AF privatized programs. This includes Asset, Acquisition and Portfolio Management services in support of HEU throughout the United States.

One of the considerations in proposal evaluation is the verification of the Offerors' past performance for Housing and Utility Privatization contracts or other work efforts which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the Offeror, for the evaluation of the Offeror's performance on those contracts or work efforts.

Our areas of interest regarding the Offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 10 calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.

To assist you in preparing your response and expediting your reply, submit the questionnaire electronically and e-mail to Mark Ingle, email: mark.ingle.2@us.af.mil and Michael I Grasso, email: michael.grasso.2@us.af.mil. Please call or email Michael I. Grasso, email: michael.grasso.2@us.af.mil, Contracting Officer at M: 210-395-8730/C; 210-347-9488, if you have any questions. Title of email: RFP FA8903-27-R-0014, Present/Past Performance Questionnaire. Your completed questionnaire will become a part of the official Source Selection records.

Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
