About this file
This is a Drug Free Workplace Certification attachment for RFP No. 26-83111500-RFP issued by the Florida Department of Management Services for Contact Center Services. The certification form is a mandatory compliance document that respondents must sign and submit with their proposals. The document outlines six specific requirements that respondents must certify their organizations have implemented: publishing a workplace statement prohibiting unlawful controlled substance manufacture, distribution, dispensing, possession, or use; informing employees of drug abuse dangers and related policies; providing employees with copies of the prohibition statement; notifying employees that compliance is a condition of employment and requiring notification of any drug-related convictions within five days; imposing sanctions or requiring rehabilitation program participation for violating employees; and maintaining good faith efforts to sustain a drug-free workplace. The certification serves as a tiebreaker mechanism—when the Department receives multiple proposals that are equal with respect to price, quality, and service, preference will be given to the respondent that has signed and submitted the certification in compliance with Florida Statutes Section 287.087.
Respondents must have an authorized company representative sign the certification form, providing their name, title, signature, and date. The form explicitly states that false statements are punishable under law, establishing the legal consequences for misrepresentation. This mandatory attachment requirement applies to all respondents and represents a compliance-based evaluation factor that can differentiate proposals when technical and cost scores are otherwise equivalent, making it a critical submission component for the statewide contact center services contract opportunity.
