About this file
The document is a Past Performance Evaluation Survey form for a federal contract opportunity (Solicitation Number 692M15-25-R-00001) for Second Level Engineering (SLE) Support Services. The survey is designed to collect reference information from previous clients about a contractor's performance across several key evaluation areas, including technical performance, cost control, schedule/timeliness, business relations, and customer satisfaction. References are asked to objectively rate the contractor's performance using a five-tier rating system ranging from "Exceptional" to "Unsatisfactory" for each performance element.
The survey instructions require offerors to complete Section A with contract details for projects completed within the past five years, and then send the form to relevant references such as Contracting Officers, Contracting Officer Representatives, or Project Managers. References are instructed to complete Section B by email to specific FAA contacts (alan.mccarty@faa.gov and christine.yezzo@faa.gov) by 2 PM EST on the proposal submission due date. The evaluation is part of a small business 8(a) set-aside competitive procurement for computer-related services at the William J. Hughes Technical Center, with the FAA intending to award a seven-year Indefinite Delivery Indefinite Quantity contract using a best value trade-off approach.
