About this file

This is a Past Performance Questionnaire template for Solicitation #030ADV26Q0054 issued by the Library of Congress. The form is designed to be completed by past or current clients who have worked with potential contractors to evaluate their performance on relevant contracts. Contractors responding to this solicitation must submit completed questionnaires from evaluators who can assess their prior work.

The questionnaire captures essential contract information including contractor name, agency contract number, prime or subcontractor status, agency name, contract type, period of performance, and total contract value. Evaluators rate contractor performance across multiple dimensions using an Excellent, Satisfactory, Poor, or N/A scale, including: quality of products or services, contract management effectiveness, timeliness of performance, deliverable quality, personnel effectiveness, quality control measures, subcontractor management, and transition/workforce retention capabilities. The form also requests narrative summaries of contractor strengths, weaknesses, future hirability, and additional comments. Completed questionnaires must be submitted via email to Jennifer Zwahlen (Contracting Officer) at jzwa@loc.gov and Jacqueline Miller (Contract Specialist) at jacmiller@loc.gov. This is an appendix/template document supporting the overall solicitation for production of a 45-minute documentary video for the Veterans History Project Anniversary with proposals due by 12:00 PM EST on 03/20/2026.
