About this file
Attachment J is a Foreign Country of Concern Attestation form (PUR 1355) required by the Florida Department of Management Services for the Contact Center Services RFP (26-83111500-RFP). This attestation must be completed and submitted by any entity bidding on or entering into a contract with the State of Florida that would provide access to an individual's Personal Identifying Information. The form requires an authorized officer or representative of the bidding entity to certify under penalty of perjury that the entity is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not maintain its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not hold a Controlling Interest in the entity. The attestation must be signed and dated by the authorized representative, with printed name and title also required.
This form is one of several mandatory compliance documents that respondents to the Contact Center Services RFP must submit as part of their proposal. Failure to submit the completed attestation would render a proposal non-responsive. The attestation is governed by Rule 60A-1.020, F.A.C., which defines the relevant terms including "Foreign Country of Concern," "Controlling Interest," and "Personal Identifying Information." Bidders should note that this requirement applies regardless of the contract's dollar value or term length and is a standard compliance measure for state procurements involving access to sensitive personal data.
