About this file
This is a Past Performance Evaluation Form from the District of Columbia Department of General Services (DGS) for the DCAM-25-CS-RFP-0014 Redevelopment St. Elizabeths East Campus Design-Build Services project, specifically related to the relocation of Historic Building 88. The form serves as a standardized assessment tool for evaluating contractor performance across multiple dimensions. Evaluators are required to assess contractor performance in five key areas: Quality of Services/Work, Timeliness of Performance, Cost Control, Business Relations, and Customer Satisfaction. Each performance element receives a numerical rating from 0 (Unacceptable) to 4 (Excellent), with an additional "++" (Plus) designation available. The form requires evaluators to provide comprehensive documentation including the evaluator's name, title, contact information, the name of the organization being evaluated, a detailed description of services provided, contract number, contract amount, and contract start and finish dates.
The evaluation guidelines establish clear performance standards and expectations for each rating category. Quality of Services/Work is assessed based on compliance with contract requirements, accuracy of reports, appropriateness of personnel, and technical excellence. Timeliness of Performance evaluates whether the contractor met interim milestones, completed work on schedule, and avoided liquidated damages. Cost Control measures whether work remained within budget, billing was current and accurate, and cost efficiencies were achieved. Business Relations evaluates the contractor's responsiveness to inquiries, effectiveness of communication, and management of subcontracting programs, particularly small and disadvantaged business participation. Customer Satisfaction addresses the contractor's flexibility, proactive solutions, and overall responsiveness to contract requirements. Evaluators must submit completed evaluations to james.marshall@dc.gov, with detailed remarks required for any ratings of Excellent or Unacceptable performance.
