About this file
This is a Past Performance Questionnaire template used to evaluate contractor performance on previous contracts similar to the current solicitation for public safety data analytics platform and support services.
The questionnaire requests references to complete detailed assessments of the Court Services and Offender Supervision Agency's (CSOSA) contractor performance on prior contracts. Completed responses must be emailed to catherine.collins@csosa.gov by 11:59 PM Eastern Time on July 2, 2026. The questionnaire gathers information from the individual most knowledgeable about the contractor's day-to-day operations, using a six-point performance scale (6=Exceptional, 1=Neutral) across five evaluation categories: Technical Performance & System Capabilities (including data ingestion, real-time processing speed, user-friendliness, intuitiveness, dashboard customization, and platform reliability); Information Security & Compliance (CJIS standards, role-based access controls, and security patch deployment); Implementation, Integration & Support (legacy system integration, training quality, and technical support responsiveness); and Project Management & Business Relations (schedule adherence, billing accuracy, and contractor flexibility). Respondents must provide contract details including reference contract number, period of performance, contract value, contractor capacity (prime, subcontractor, or key person), and narrative explanations for any performance ratings below 4. References are encouraged to supplement individual responses with observations from others within their organization. The document notes that if the contractor holds multiple active or recent CSOSA contracts, separate questionnaires should be completed for each contract within the past three years.
