About this file
This is a Past Performance Questionnaire template used by the Library of Congress to evaluate contractor suitability for solicitation 030ADV26R0049, the Audio System Modernization project for the Coolidge Auditorium. The questionnaire requests assessment of a contractor's performance on previous projects through a standardized rating scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) across ten evaluation criteria including overall contract management, product and service quality, project phase management, schedule compliance, report timeliness, subcontractor performance, key personnel competency and communication, cost performance, and contract requirement fulfillment. Respondents must provide general company information, contract details, and narrative explanations for any Marginal, Unsatisfactory, or No ratings.
Completed questionnaires were required to be submitted via email to Olexiy Levchuk (olevchuk@loc.gov) and Moji Adejuwon (moad@loc.gov) no later than 12:00 PM Eastern on August 25, 2026. The questionnaire emphasizes that past performance is of considerable importance to the Library's acquisition decision and requests honest assessments from current or previous customers of the contractor being evaluated. Handwritten responses are acceptable, and respondents are encouraged to supplement their knowledge with input from others in their organization.
