About this file

This is a Conflict of Interest Affidavit required for submission with bids/proposals on a Maryland Department of Transportation State Highway Administration (MDOT SHA) contract for overhead door maintenance at the SHA Elkton Shop in Cecil County. The affidavit establishes definitions and requirements pertaining to conflicts of interest for offerors, contractors, consultants, subcontractors, and their employees or agents who have authority to control or supervise contract work. The offeror must warrant that no relevant facts or circumstances create a conflict of interest, except as disclosed in the affidavit, and must immediately notify the procurement officer in writing if an actual or potential conflict of interest arises after submission. The affidavit requires the authorized representative and affiant to solemnly declare under penalties of perjury that all contents are true and correct, with signature and date fields provided for execution.

The underlying contract opportunity is a three-year blanket purchase order for maintenance services on 32 overhead doors ranging from 8' x 10' to 20' x 13' across three MDOT SHA facilities in Cecil County. Pricing consists of three components: hourly rates for maintenance work performed during normal business hours (with separate rates for mechanic/technician and helper, requiring SHA preapproval for two-person jobs); emergency breakdown/after-hours pricing for calls after 3 p.m. or weekends; and a lump sum cost for semi-annual inspection and light preventative maintenance on all 32 doors (six total services over the three-year period). Bidders must select a materials and supplies markup between 0% and 10% gross margin, with SHA reserving the right to request receipts or commercial invoices for verification. No subcontracting is permitted, and the contract scope has been limited to overhead doors only, excluding man-door maintenance from the previous contract version.
