About this file
This is a Conflict of Interest Affidavit form required by the Maryland State Archives for the Generator Preventive Maintenance & Cellular Monitoring System procurement at two locations in Maryland. The affidavit, effective July 2024, requires all offerors, contractors, consultants, subcontractors, and their employees or agents with authority to control or supervise work to warrant that no conflicts of interest exist that would impair their ability to render impartial assistance or advice to the State or compromise their objectivity in performing contract work. Offerors must disclose any relevant facts or circumstances that give rise or could give rise to conflicts of interest, and must immediately notify the procurement officer in writing of any actual or potential conflicts that arise after the affidavit is submitted. The contract pertains to quarterly preventative maintenance and cellular monitoring system services for generators at the Maryland State Archives' Annapolis and Windsor Mills campuses, with a base term of one year and two optional one-year renewal periods.
The affidavit must be submitted with the bid or proposal and is executed under penalties of perjury by an authorized representative. The document requires disclosure of any circumstances that could create competitive advantages or impair objectivity, with space provided for detailed explanations and additional supporting documentation. If a contract is awarded and performance has commenced, the contractor shall continue performing its obligations until the procurement officer provides contrary direction regarding the identified conflict.
