About this file

Attachment H is a detailed quarterly performance measures and deliverables report format document for the Florida Department of Management Services (DMS), specifically for the State University System Optional Retirement Program (SUSORP) and Senior Management Service Optional Annuity Program (SMSOAP). The document provides a comprehensive matrix of performance metrics, including deliverables, service levels, financial consequences for non-compliance, and reporting requirements across multiple operational areas such as member services, contributions, distributions, website management, call center performance, authorized products, reporting, and contract documentation.

The performance measures are structured with specific required service levels typically set at 100%, with financial penalties ranging from $100 to $15,000 per occurrence or per business day of delay. Key performance areas include timely submission of reports, accurate member notifications, processing of member transactions, maintaining website and call center functionality, managing authorized products, and ensuring compliance with regulatory requirements. Each metric includes a mechanism for tracking performance status, number of occurrences, delinquent days, and potential financial consequences, creating a robust accountability framework for the service provider. The document serves as a template for quarterly reporting and performance evaluation, allowing the DMS to systematically monitor and enforce contract performance standards.
