About this file

This is a Responsibility Inquiry form for OregonBuys Bid #S-62900-00015115 related to a Malheur National Forest Magone GNA PCT and Piling project. The document serves as an assessment tool that the procuring agency will use to determine bidder responsibility prior to contract award and execution. The form requires bidders to demonstrate their capability to meet all contractual responsibilities through documentation of appropriate financial, material, equipment, facility, and personnel resources. Bidders must disclose their contract performance history over the past five years, including the number of similar contracts completed on time and within budget without claims, as well as those that failed to meet these standards.

The inquiry addresses several responsibility factors including criminal or civil charges within the past three years related to public contracts, antitrust violations, or crimes such as embezzlement, theft, forgery, and bribery. Bidders must also disclose contract terminations for default, lawsuits involving creditors or contract disputes, outstanding judgments, and any financial distress or cash flow concerns within the past three years. Additionally, bidders must confirm current licenses, insurance, registrations, and legal authorization to conduct business in the State of Oregon. Bidders receiving services contracts must possess a current, unexpired Pay Equity Certificate issued by the Department of Administrative Services, unless such certification was already provided with the original bid submission. An authorized representative must sign the form, certifying that all responses are complete, accurate, and not misleading.
