About this file
This document is a Fair Housing and Equal Opportunity Certification form for the Long Beach Continuum of Care (CoC), a federally-funded homeless assistance program. The certification establishes the compliance framework and operational requirements for all projects receiving federal financial assistance through the CoC program. Projects must operate in accordance with specified timelines: non-rental assistance projects involving acquisition, rehabilitation, or new construction must operate for a minimum of 20 years from the date of initial occupancy or service provision, while projects receiving assistance solely for supportive services, leasing, or operating costs must operate for any year in which such assistance is provided.
The certification requires applicants to comply with numerous federal civil rights statutes and executive orders, including Title VI of the Civil Rights Act of 1964, the Fair Housing Act, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and Executive Orders 11063, 11246, 11625, 12432, and 12138. Applicants must ensure non-discrimination in all program activities based on race, color, religion, sex, disability, familial status, national origin, and age; provide reasonable modifications and accommodations; establish outreach procedures to reach qualified persons of underrepresented groups; and take affirmative action to encourage participation by minority-owned and women-owned businesses. Rental assistance projects with established preferences for disabled persons must maintain nondiscrimination requirements within the designated population. An authorized agency official must sign the certification, acknowledging awareness that false statements may result in criminal, civil, or administrative penalties under U.S. Code Title 218, Section 1001. Applicants unable to certify to any statement must provide a written explanation.
