About this file
This is a Responsibility Inquiry form for ITB S-DASOBO-00015789, issued by the Department of Administrative Services (DAS) State Procurement Services in Oregon for USDA Food Carrier and Warehouse Services. Bidders must complete and return this form with their bid submissions. The form requires bidders to demonstrate their financial, material, equipment, facility, and personnel resources necessary to fulfill contractual responsibilities. DAS SPS will evaluate bidder responsibility prior to contract award and may request additional documentation including profit-and-loss history, balance statements, cash flow information, assets-to-liabilities ratios, bonding capacity, insurability, credit information, facility capabilities, personnel qualifications, and performance records from previous contracts. Failure to promptly provide requested information or demonstrate responsibility may result in rejection of the bid.
The Responsibility Inquiry addresses multiple evaluation criteria designed to assess bidder qualifications and financial stability. Bidders must disclose their performance history on similar contracts completed within the last five years, including the number of contracts completed on time and within budget with no claims and the number that failed to meet those standards. The form requires disclosure of any criminal or civil charges, indictments, or convictions within the last three years related to public contracts, antitrust violations, or crimes such as embezzlement, theft, forgery, or bribery. Additionally, bidders must report contract terminations for default, lawsuits involving creditors or contract disputes, outstanding judgments, financial distress, cash flow concerns, bankruptcy filings, and reorganization actions within applicable timeframes. Bidders must confirm possession of all required licenses, insurance, and registrations to conduct business in Oregon and provide an unexpired Pay Equity Certificate issued by the Department of Administrative Services where applicable.
