About this file

This document is a Past Performance Questionnaire template used by the National Geospatial-Intelligence Agency (NGA) for solicitation HM047626R0029. The questionnaire is designed to be completed by both offerors and evaluating organizations to assess contractor performance on federal contracts.

The offeror section requires submission of basic contract information including contractor name, contract number, contract value, contracting officer and program manager details, award and completion dates, and contract type. Offerors must provide a summary description of the contract work and identify the percentage of cleared personnel turnover at contract completion. Additionally, offerors must indicate whether the contract involved classified work and specify the classification level if applicable (Department of Defense Collateral, Sensitive Compartmented Information, Secret, Top Secret, or other Special Access Programs). The evaluating organization section collects evaluator contact information and the observed performance period.

The performance evaluation component uses a 10-point rating scale (0-10) with categories ranging from Neutral (No Score) through Unacceptable, Marginal, Acceptable, Good, and Outstanding. Evaluators assess contractor performance across three primary categories: Management Performance (personnel delivery/retention and program management), Technical Performance (technical approach and quality assurance), and associated competencies such as resource prioritization, domain experience, deliverable quality, personnel expertise, milestone achievement, cost management, and security violation reporting. The form requires written comments explaining ratings of Outstanding, Good, Marginal, or Unsatisfactory, and includes questions regarding whether unsatisfactory performance was communicated to the contractor and whether the evaluator would recommend rehiring the contractor. Completed questionnaires must be returned to Kenneth M. Lehrmann (Contracting Officer) and Janet D. Richards (Contracting Specialist) at NGA.
