About this file
This is a Substitution Form (Attachment D) for a cooling tower refurbishment project at the County Service Complex in Merritt Island, Florida. The form permits contractors to propose equivalent items or exceptions to the specified materials and equipment required for the project, which involves furnishing labor, materials, tools, equipment, and transportation for removal and replacement of structural steel components, cooling tower fill, cooling tower fan motor, and pump motors at 2575 North Courtenay Parkway, Merritt Island, Florida 32953. Contractors proposing substitutions must include the item number, item description, manufacturer's name, model number, packaging quantities, and supporting documentation such as cut sheets or data sheets for each proposed equivalent product.
The form establishes that contractors supplying no exceptions to the specifications must provide all materials exactly as specified, and any proposed equivalent or exception deemed a material deviation from specifications will result in mandatory bid rejection. Contractors are advised that submission of proposed equivalents is entirely at their own risk and that this form cannot be used to attach conditions, limitations, or other provisos to the bid. The form requires completion with company name, address, telephone, fax number, and authorized signature before submission.
