About this file

This is an Organizational Conflict of Interest certification form that must be completed for each task order by a firm officer.

The form requires the contractor or subcontractor to disclose whether an organizational conflict of interest exists for the specific task order, with space provided to describe any identified conflicts. The firm must acknowledge all contract restrictions and agree to execute additional agreements as directed by the Contracting Officer. The form certifies that four specific safeguard measures are in place: a "fire wall" with personnel advised of restrictions per FAR 9.505-4; electronic safeguards preventing unauthorized document access; physical safeguarding and securing of contract-related documents; and an agreement with offerors to protect their information from unauthorized use or disclosure and restrict its use to the purpose provided. The form requires signature, printed name, date, and title from a firm officer and includes blank fields for contractor name, subcontractor name, contract number, and task order number.
