About this file

This is a Past Performance Questionnaire (PPQ) form used to collect references on contractor performance for solicitation 36C10F26R0045. The form is designed to be completed by past or current customers of contractors bidding on the opportunity and should be submitted via email to Kevin Armillotti, David Reich, and Fritz Fabien at the Department of Veterans Affairs with the solicitation number, offeror name, and "Completed Past Performance Questionnaire" in the subject line.

The questionnaire gathers detailed information about the contractor's company, point of contact, address, and specific contract details including contract number, type, technical complexity, period of performance, total contract amount, and a description of services provided. The evaluation section uses a four-point rating scale (1=Unsatisfactory, 2=Marginal, 3=Acceptable, 4=Very Good, with an additional rating of 5=Outstanding, and N/A for not applicable) with detailed adjective definitions for each rating level. Evaluators assess twelve performance criteria: schedule and requirement conformance, management capabilities and personnel replacement, supplier quality and dependability, quality control effectiveness, problem identification and alternative solutions, communication and clarification requests, emergency response capability, pricing accuracy and timeliness, invoice accuracy, cure notices or payment withholding, warranty defect resolution, and contractor cooperation and customer satisfaction commitment. The form includes space for comments and remarks on each criterion and concludes with an optional section for additional evaluative comments.
