About this file
This document is a detailed procedure guide for the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program for healthcare providers serving veterans. The guide outlines a six-step process for providers when they are notified about a high-risk veteran: (1) receive notification from a coordinator, (2) optionally access a dashboard, (3) re-evaluate the veteran's care through comprehensive electronic health record review, (4) consider treatment enhancement strategies, (5) contact the veteran using specific talking points, and (6) document interactions in the CPRS system.
The procedure emphasizes comprehensive care review, including rescreening evaluations, reviewing diagnoses and treatment plans, and ensuring veterans have access to needed services. Treatment enhancement strategies include enhanced communication, safety planning, increased monitoring of stressful life events, and interventions to improve coping strategies. Providers are instructed to complete all tasks by the fourth Wednesday of the month and to reach out to identified veterans within one week of notification, using a national provider note template to document interactions.
