This is in addition to the PCMHI staffing ratio. PCMHI does not count towards the general/specialty mental health staffing requirements.
47
The RFP states, required: Administrative Support Staff: FTE Ratio Performance Standard: Current minimum standard is 1.0 FTE administrative staff per 1,000 Veterans receiving General and Specialty MH services.
VHA Outpatient Mental health staffing policy currently requires 1.0 MH administrative support staff per 1,000 MH patients. With 507 patients in MH CLIN, is it appropriate for offerors to include 0.5 MH FTE administrative support staff instead of the 1.0 FTE outlined in the RFP?
This can be shared, but there needs to be administrative support available throughout the entire day (e.g. it cannot only be on Monday, Wednesday and Friday). Patients will need to be checked in/checked out/scheduled for follow-up, etc.

The RFP states, evidence of past performance shall be submitted to the Contracting Officer directly prior to the close of the solicitation period.
In the Instructions to Offerors, it states Proposals will consist of submitting two separate proposal volumes: Volume I - Price and Volume II - Technical Capability, Business Operations, and Small Business Participation. There is no mention of providing Past Performance information.
In Attachment D.6 Deliverables / Reports, it states, a minimum of three (3) past performance references, utilizing the Past Performance Questionnaire.
Attachment D.19 Past Performance Survey was provided.
There is no mention of Past Performance in the Instructions for the Preparation of Proposal. Given that Volume II can not contain any vendor (Offeror) specific information, rather the Vendor will obtain a code from the Contract Specialist, Angela Tucker, and that code shall be utilized in Volume II in place of Vendor's name, would the Past Performance references be located in Volume I Price?
Is there any information or format the VA would like to see in the Past Performance references?
Reference above answer to question 1 which states to include the Past Performance references and relevant information in Volume I, as Section 4.
There is no set format for Past Performance references. It is recommended to include an active point of contact. As a reminder, all Past Performance Surveys/Past Performance Questionnaires shall be submitted by the reference and not the offeror.

Yes, contractor shall provide all medications, including any necessary vaccines that are to be administered to patients in the clinic. To date, Savannah clinic has not had to prescribe any.
51
The RFP states, the Contractor will provide all medications that are to be administered to patients in the clinic (including any necessary vaccines, with the exception of vaccines for COVID-19 as indicated in section 4.6.3.4). (Remove “with the exception of vaccines for COVID-19 as indicated in Redistribution of COVID-19 Vaccine to Contracted Outpatient Sites of Care” if that section is deleted. Please remove this instruction prior to submitting). Facilities must collaborate with National PBM to request a waiver when they prefer to provide medications, vaccines, and PYXIS machines to contracted clinics. If waiver is granted, a copy of it should be submitted with your technical and clinical review documents. Remove this instruction prior to submitting. Routine prescriptions will be dispensed by the VA and mailed via VA Centralized Mail Out Pharmacy (CMOP) to the Veteran following appropriate Contractor’s provider order entry in the VA EHR.
Will the VA provide a complete list of medications and vaccines to include all injectable medications that the Contractor is expected to provide? And, will the VA provide historical monthly and annual usage at the Savannah CBOC for the past 3 years?

53
The RFP states, The Contractor shall meet all requirements for anticoagulation management outlined in VHA Directive 1108.16 Anticoagulation Therapy Management (or subsequent revisions thereto) as well as local policy related to the management of patients on anticoagulation. The RFP also states, the Contractor shall provide Quarterly and annual anticoagulation quality assurance summaries as outlined by the local Pharmacy & Therapeutics Committee.
If the VA is providing the Clinical Pharmacy Services, wouldn't the VA be responsible for the anticoagulation management and providing anticoagulation quality assurance summaries?
Yes, VA provides the AC services listed.
54
The RFP states, Clinical Pharmacy Services: Clinical Pharmacy Services shall be provided by the Memphis VAMC. The contractor shall provide space as detailed in space requirements, support staffing, and ancillary support to allow for the provision of clinical pharmacy services. The support services shall be consistent for the CPS as with other prescribers for each Contractor’s scheduled clinic.
Is it acceptable for the PACT Team staff to provide support for the VA provided CPS?
For the PACT CPP, we do not need ancillary support at the site (i.e. MSA, lab and occasionally nursing).
55
The RFP states, the contractor shall provide by way of contract through a local accredited and certified facility at the contractor’s expense for general x-rays, diagnostic radiographs and mammography services (screening mammography, diagnostic mammography and breast ultrasound).
Does the VA have historical data on general x-rays, diagnostic radiographs and mammograms completed by the Savannah CBOC?
FY24 85 exams 1 MRI, 13 U/S, 71 MAMMO FY 25 104 exams 5 MRI, 2 BIOPSY, 16 U/S, 81 MAMMO

Covid testing.
57
The RFP states, there will be (1) room a minimum of 125 square feet with carpeted flooring, upper cabinetry with lower workstation on one wall, a desk that is a minimum of 3 feet wide, (3) chairs, and a total of (4) network access ports with port (1) located on each wall inside the room for the Memphis VAMC to utilize for a VA staff Audiologist during normal business hours. The room will have a lock on the door with keys only kept by the VA, office manager, and housekeeping. In order to avoid disruption of care, the room shall not be located beside an outside door or emergency door. The VA will provide and be responsible for the equipment needed for the ophthalmology and audiology clinics. The Contractor will be responsible for maintaining the rooms. The VAMC may utilize these two rooms for other specialty needs in place of or in between Audiology/Ophthalmology clinic operating days/hours. It is anticipated that both rooms will be utilized by the VA for the duration of the contract.
Does the VA want 1 or 2 additional rooms? Would the VA be willing to receive alternative proposals for offerors to provide these 2 clinic services instead of the VA?
VA doesn't currently need space for Audiology at this time. Tele-audiology services will be provided by VA.
58
Will pharmacy services be available at the Clinic? If so, will it be thru Tele-Pharmacy or clinical Pharmacist? If so will a Tele-Pharmacy room be needed or private office?
Pharmacy services will be provided through tele pharmacy. It is anticipated the VA will utilize Telehealth Primary Care for up to 40 hours per week.

62
Does the require the two Tele-health TCT to have offices or workstations?
The TCT can have a workstation, however they do test calls/VVC so this needs to be in a quiet area.
63
Does the VA require a private office for the Podiatry Consultant?
No.
64
Does the VA require a private office for both the Retinal Camera room and Tele-Retinal room?
The retinal camera room and the Tele-Retinal room are the same room.
65
Does the VA require a private office for both the Tele-Health Audiology and Tele-Retinal Opthamology?
Yes, the tele audiology and the tele-retinal require their own room. Tele Audiology needs to be in an area that provides quiet, the equipment is very sensitive and can pick up false noise. Away from a main hall, and areas where Veterans are constantly present is ideal. Typically, at the end of a hall is better.
66
Does the VA require Tele-Mental Health rooms? If so, how many?
Yes, we are required to have Tele Mental health rooms for warm hand-off patients. This room would house a computer with a web cam or a Cas cart/ The Cas carts are pretty big.

In Evaluation Factor 1 - Past Performance, it states, Offeror shall submit at least three (3) references for current/prior Outpatient Clinic and Healthcare Management contracts that are similar in size and scope to this requirement, including any Federal, State, or local government, and commercial contracts. The Government will consider Offeror’s record for delivery, quality and past or present malpractice judgments, and proposed major subcontracts as it relates to the probability of successfully performing the solicitation requirements. Past performance evaluations will be conducted using information obtained from any sources deemed appropriate. While the Government may elect to consider data obtained from other sources, the burden of providing current and complete past performance information rests with the Offeror. The past performance information obtained will be used for both the responsibility determination and this evaluation factor. Offerors without a record of relevant past performance or from whom information on past performance is not available will not be evaluated favorably or unfavorably. Offeror shall also submit with proposal its overall turnover rate of professional personnel working under the same or similar contracts. (PWS Reference: 2.21).
Is the overall turnover rate that the VA is referencing in Evaluation Factor 1 - Past Performance, referring to the overall turnover rate for the reference contract provided by the bidder, or is it referring to the bidders overall company turnover rate for same or similar contracts?
