About this file

This document is a Department of Veterans Affairs (VA) internal guidance document specifying which healthcare staff members are authorized to develop Suicide Prevention Safety Plans and complete Suicide Behavior and Overdose Reports (SBOR). The document provides a detailed matrix of healthcare professional credentials indicating who can perform these critical suicide prevention tasks, with credentials such as Medical Doctors, Licensed Psychologists, Clinical Pharmacist Practitioners, Social Workers, Marriage and Family Therapists, Mental Health Counselors, Addiction Therapists, Registered Nurses, Advanced Practice Registered Nurses, and Physician's Assistants being predominantly approved.

The guidance outlines two key requirements for staff to develop safety plans or complete SBORs: 1) having a "Yes" designation next to their specific professional credentials, and 2) obtaining local facility Chief of Staff concurrence and ensuring the task is within their facility's scope of practice and competency. The document also notes that trainees in approved credential categories may complete these tasks with an appropriate co-signer. Notably, some roles like Licensed Practical Nurses, Peer Support Specialists, Unlicensed Assistive Personnel, Psychological Technicians, Social Work Assistants, and Vocational Rehabilitation Specialists are explicitly prohibited from developing safety plans or completing SBORs.
