RFP No. S-19800-00014538 – Veterans Treatment Court Program Expansion Consulting Services
This document is a Proposer Certification Sheet for the Oregon Department of Justice (OJD) related to consulting services for Veterans Treatment Court Program Expansion. The certification sheet is a required component of the proposal submission process and outlines the mandatory certifications and representations that proposers must make to be considered for contract award. Proposers must certify their understanding and acceptance of RFP requirements, acknowledge receipt of all addenda, and confirm that their proposal remains firm for 180 days from the closing date. If awarded, proposers agree to perform the scope of work and meet performance standards set forth in the final negotiated statement of work. The RFP specifies that contract terms and conditions are outlined in Attachment A and may be modified by addenda, except for those items OJD has reserved for negotiation.
The certification sheet contains fourteen specific certifications covering tax compliance, non-discrimination in employment practices, wage equity, compliance with broadband Internet access service laws, exclusion list verification, conflict of interest disclosure, and attestation to the truthfulness and accuracy of proposal contents. Proposers must certify that they and their employees, agents, and subcontractors do not appear on the Treasury Department's Specially Designated Nationals and Blocked Persons list or the System for Award Management government-wide exclusions lists. Additionally, proposers must certify compliance with Oregon Revised Statutes regarding employment discrimination, wage payment practices, sexual harassment prevention policies (required for contracts valued at $150,000 or more), and that all proposal statements are truthful and prepared independently without collusion or fraud. The certification sheet serves as a binding legal document and any false or fraudulent statements may constitute violations under the Oregon False Claims Act.
