This document is Attachment C to contract DMS-25/26-032 for Irrigation System Management Services. It establishes the performance standards and measurement criteria that the Contractor must meet on a quarterly basis, with the Facility Manager required to complete and submit the form quarterly to the Contract Manager. The form requires the signatures of both the Facility Manager and Contractor upon completion.
The document specifies four key deliverables with associated performance guarantees and measurement standards: (1) Damage, Leaks, and Malfunctions—the Contractor must immediately report any irrigation system damage, leaks, or malfunctions via email to the DMS Facilities Manager with a 5% invoice deduction for noncompliance; (2) Minor Repairs—the Contractor must repair minor irrigation system components damaged during routine service operations with a 5% invoice deduction for noncompliance; (3) Irrigation Schedule—the Contractor is responsible for irrigation operations and schedule with prior approval required from the DMS Facilities Manager for any deviations, subject to a 10% invoice deduction for noncompliance; and (4) Quarterly Summary Report—the Contractor must submit Attachment B within five business days of the quarterly inspection with a 5% invoice deduction if the report is not timely submitted. Performance is rated on a three-point scale: 1-Good (Performance Acceptable), 2-Fair (Performance Needs Improvement), and 3-Poor (Below Standards/Non-Compliant), with invoice deductions applied for Poor ratings.
