ATTACHMENT BB - MONTHLY REPORT SUMMARY

This document establishes monthly reporting requirements for contractors awarded litter and encampment abatement services by the City of Long Beach under Request for Proposals PW-25-525. Contractors must submit consolidated monthly reports by the specified deadline, covering all projects completed and invoiced during the prior month in a single PDF file. The monthly report must include invoice tonnage weight with monthly totals, individual invoice amounts with monthly total amounts, and detailed backup documentation for each job consisting of before-and-after photographs and tonnage receipts from disposal facilities.

All invoiced costs must be comprehensive and all-inclusive, encompassing direct and indirect expenses for labor, overhead, incidental supplies, travel, mileage, and fuel. Special materials or supplies may only be purchased by the contractor after written discussion and authorization from the City projects manager or designee. The document includes a template table (Attachment BB-1) for organizing invoice data by date, invoice number, invoice amount, and tonnage weight, with rows for individual invoices and a totals row highlighted in yellow for monthly summary purposes.
