About this file

This document is an insurance requirements schedule issued by the State of Nevada's Risk Management Division for standard professional service contracts. The schedule establishes mandatory insurance coverage that contractors and subcontractors must procure and maintain throughout contract performance and any applicable warranty periods. The requirements include Commercial General Liability with a general aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Automobile Liability with a combined single limit of $1,000,000 for owned, hired, and non-owned vehicles; Workers' Compensation at statutory levels with employers' liability limits of $100,000 per accident and $500,000 policy limit for disease; and Professional Liability (Errors and Omissions) with per-claim limits of $1,000,000 and annual aggregate of $2,000,000. All policies must name the State of Nevada as an additional insured and maintain primary and non-contributory coverage status.

All insurance policies must be placed with insurers licensed in Nevada and maintaining an A.M. Best rating of not less than A-VII. Contractors must provide certificates of insurance on ACORD forms or state-approved equivalents prior to work commencement and maintain coverage for the contract duration. The State requires thirty days' prior written notice of cancellation, except for non-payment situations which require ten days' notice. Failure to maintain required insurance constitutes material breach of contract. Contractors may utilize excess or umbrella policies on a following-form basis to satisfy minimum requirements, and the State reserves the right to require certified copies of complete policies at any time. All subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates of insurance meeting identical minimum requirements. Modifications to these insurance requirements require approval from the State's Risk Management Division or Attorney General's Office.
