About this file
This document is a Proposers Certifications and Representations form for Request for Proposal (RFP) Number 6.13.25.1 issued by the David Douglas School District for Garbage, Food Waste and Recycling Hauling Services. The proposal allows for pricing to be guaranteed for one year with options to renew for up to six additional 12-month periods. Price increases would require 30 days advance notice, and the district would receive immediate benefit of any price decreases.
The proposer must comply with various state regulations, including non-discrimination requirements, construction contractor board registration (if applicable), and prevailing wage standards. The document specifies that for contracts over $50,000 involving construction services, painting, or fixture installation on district property, prevailing wage rules apply. The proposal does not require proposal security, performance bond, or payment bond, and is not subject to prevailing wage requirements. Proposers must certify they have not discriminated against minority, women, or emerging small business enterprises, and must disclose their business designation and resident bidder status.
