Sacramento County Department of Airports SMF Pedestrian Walkway Riparian Public Art Installation Project

ATTACHMENT A

MINIMUM CONTRACTOR QUALIFICATIONS

CUPCCAA Informal Bid | Contract No. 41016 | November 2026 Purpose and bid submission The Bidder shall submit a Contractor Qualification Statement, signed Self-Performance Certification, and the supporting information required by this Attachment with its Bid. The minimum qualifications apply to the Contractor-assigned Riparian artwork-installation Work described in the Special Provisions.

The County may reject a Bid as nonresponsive if material required qualification information is omitted, or determine a Bidder nonresponsible if the submitted information does not demonstrate the minimum qualifications. The County may request clarification or verification of timely submitted information. Clarification shall not cure an omitted material qualification, add a construction subcontractor, substitute another entity’s experience for the Bidder’s required experience, materially change proposed key personnel or qualifications, or otherwise materially revise the Bid after opening.

1. LICENSE AND REGISTRATION

The Bidder shall hold, at the time required by California law and throughout performance of the Work, an active and valid California C-61/D-34 — Prefabricated Equipment Contractor license as required by Special Provisions Section 2-5. A Class B — General Building Contractor license without the required C-61/D-34 classification does not satisfy the stated requirement. The Contractor’s classifications must be legally sufficient for all Work it performs. Identification of C-61/D-34 in the bid documents does not authorize work outside the lawful scope of the Contractor’s license.

Contractor-assigned Work includes authorized onsite receipt, joint condition inspection and inventory, custody, uncrating, sorting and registration control, staging, protection, installation, alignment and orientation of fragile glass rondelles and supplied artwork-side rods, clips, connectors and hanging hardware on a completed and released supporting grid; initial Artist-coordinated tuning; inspections, correction, cleanup, record documentation and closeout. Installation and access equipment and qualified operators are Contractor-furnished.

Unless expressly added by written Addendum before bid opening or an authorized written Contract change after award, and legally within the Contractor’s classifications, the Work excludes:

· Artwork fabrication and transportation to the designated SMF receiving location.

· Design, fabrication, installation, repair or alteration of the permanent ceiling suspension, supporting grid, wire-mesh panels and primary structural-support infrastructure.

· Permanent structural steel fabrication or erection, permanent structural welding, structural reinforcement, and permanent building modification.

· Permanent-design engineering for the artwork, support system or building.

· Electrical work and glazing or curtain-wall modification.

· Brass portal-column artwork, brass floor inlays and terrazzo.

· Correction of work performed by others and other artwork installations.

The Contractor shall not introduce a means, method, temporary system or installation approach requiring separately licensed construction Work outside the lawful scope of its license. The movable-scaffolding and other access requirements remain subject to Sections 2-5 and 2-8. Any licensing concern shall be submitted through the required bid-question or RFI process; County direction does not authorize unlicensed Work or construction subcontracting.

· Provide Contractor legal name and business address.

· Provide California contractor license number, classifications held and expiration date.

· Provide Department of Industrial Relations public works contractor registration number.

· Certify that required licenses and DIR registration will be maintained at all times required by law and the Contract.

The County may verify license and registration status with CSLB and DIR.

2. SELF-PERFORMANCE

The Contractor shall self-perform one hundred percent (100%) of all field construction and artwork-installation Work using employees directly employed and paid by the Contractor and under its direct supervision and control. Construction subcontractors and lower-tier construction subcontractors are prohibited. The Bidder shall mark “None” on the County designation-of-subcontractors form and any other required construction-subcontractor listing.

Material and equipment suppliers, common carriers, equipment-rental companies, manufacturer representatives, licensed professional engineers and independent testing or inspection agencies are not construction subcontractors for this requirement only when they do not perform field construction or artwork-installation labor. Permitted professional services may include required temporary-works and means-and-method engineering. The Contractor remains responsible for their products, services and information.

All field equipment operators, installation workers and personnel performing rigging shall be qualified Contractor employees. Rental-company operators or other outside field crews shall not substitute for the Contractor’s own employees. Artist observation and aesthetic direction shall comply with Section 4-1.03(D) and do not replace Contractor installation labor.

The Bidder’s signed Self-Performance Certification shall confirm:

· All field construction and artwork-installation Work will be performed by the Bidder’s own employees.

· No construction subcontractor or lower-tier construction subcontractor will be used, including after award.

· The Bidder has sufficient direct workforce and supervision to perform the required installation approach and complete the Work within the Contract Time.

· All installation and access equipment is Contractor-furnished, with qualified Contractor-employed operators.

· The Bidder will comply with the self-performance requirements throughout the Contract.

This is a material Contract requirement. Unauthorized subcontracting may result in Bid rejection or the remedies stated in the Special Provisions and applicable law.

3. COMPARABLE PROJECT EXPERIENCE

The Bidder shall have successfully completed at least three (3) comparable projects within the preceding five (5) years involving suspended public art, museum or exhibit installation, architectural-feature installation, rigging-intensive installation, or comparable specialty installation work. At least one project shall have included overhead or suspended installation above or adjacent to finished public, occupied or sensitive areas. At least one shall have been in an airport, transportation facility, museum, civic/public facility, active commercial facility or similarly sensitive finished environment.

The Bidder shall have directly performed the experience claimed. Experience belonging solely to a subcontractor, supplier, consultant, affiliate, parent company, sister company or other separate legal entity does not satisfy the Bidder’s experience requirement.

For each project provide:

· Project name and location; owner/client name.

· Completed scope and the Bidder’s directly performed scope.

· Contract value and completion date.

· Owner/client reference name, telephone number and email.

· Explanation of similarity, including overhead work, fragile components or finishes, installation coordination, and facility protection as applicable.

The County may contact references and verify the submitted information.

4. SPECIALTY ARTWORK-INSTALLATION EXPERIENCE

Identify the comparable projects that demonstrate direct experience in the following applicable activities:

· Receiving, inspecting and documenting the condition and inventory of sensitive components.

· Uncrating, handling, staging and protecting fragile artwork, exhibits or comparable prefabricated components.

· Maintaining component identification, registration and placement information.

· Assembling and installing prefabricated components using the supplied compatible hardware.

· Planning and performing required elevated access, rigging and lifting.

· Confirming location, attachment, orientation, alignment, spacing and clearance.

· Coordinating Artist or designer review and initial adjustments without unauthorized design changes.

· Completing inspection, correction, records, warranty and closeout requirements.

5. Workforce rigging access equipment and qualifications Provide a direct-workforce and supervision plan identifying proposed key employees, their roles and relevant experience, and the Bidder’s capacity to perform the baseline two concurrent installation teams or a reviewed alternative permitted by Section 4-1.03(B). Identify the proposed Project Manager, site supervisor and installation team leads.

Demonstrate training, licenses, certifications or documented qualifications appropriate to the proposed activities, including:

· Movable-scaffold assembly, inspection, movement and use, as applicable to the selected system and lawful scope.

· Scissor-lift and other proposed access-equipment inspection and operation.

· Rigging, suspended-load control, signal-person and spotter responsibilities where applicable.

· Elevated work, fall protection and rescue.

· Safe fragile-glass handling and the proposed installation methods.

The Bidder shall demonstrate with its Bid the direct workforce and organizational capacity to provide the required qualified personnel. Equipment-specific operator and training documentation for the actual selected equipment may be submitted in the Rigging, Lift, Equipment and Qualification Package under Item 4 of Attachment B, before the affected use. The Contractor selects and furnishes the equipment; there is no County-furnished equipment or operator allowance.

The Bidder may not rely on an equipment-rental-company operator, construction subcontractor or separate entity to satisfy a qualification assigned to its own field employees.

6. FINISHED FACILITY PROTECTION EXPERIENCE

Identify relevant completed projects and describe the methods used to protect finished flooring and terrazzo, joints and thresholds, glazing and curtain walls, walls and ceilings, doors and architectural finishes, public circulation and adjacent occupied or sensitive areas. Address equipment movement, load distribution and the protection of fragile components.

7. SAFETY QUALIFICATIONS

Provide the Bidder’s experience-modification rate for each of the most recent three years for which available; identify any serious or willful OSHA or Cal/OSHA citations and workplace fatalities during the preceding five years; and briefly describe corrective actions. Explain information that is not available or not applicable.

Demonstrate ability to provide the required IIPP, Code of Safe Work Practices, Contract Specific Safety Plan and applicable task-specific safety plans. Project planning shall address fragile-glass handling, overhead work, elevated access, equipment movement, rigging where used, fall protection and rescue, controlled work zones, emergency response and airport coordination. These disclosure requirements do not establish an unstated numerical safety pass/fail threshold.

8. PROJECT COORDINATION

The Contractor shall demonstrate ability to coordinate with the County, Department of Airports, Project Manager, Project Inspector, Airport Operations, Airport Security, CMID/DGS, the Artists or their representatives, County-designated design professionals, adjacent contractors, commissioning teams and other County-authorized representatives. The County contracts directly with the installation Contractor. Artist direction does not independently change Contract Price or Contract Time.

9. BONDS AND INSURANCE

Demonstrate ability to provide the required performance and payment bonds, insurance, endorsements and other compliance documents in the amounts, forms and periods required by the Agreement, Special Provisions and final County-approved insurance requirements. Fine-art specialty work does not waive these requirements.

10. REQUIRED QUALIFICATION SUBMITTAL

Submit the complete Contractor Qualification Statement, Self-Performance Certification and supporting information with the Bid. The three-Working-Day post-bid submission timing in SCS Section 2-6 does not apply to this Project-specific submission:

· Legal name, contact details, license classifications/number/expiration and DIR registration.

· Signed Self-Performance Certification addressing Section 2.

· County designation-of-subcontractors form marked “None.”

· Three comparable project references and information required by Section 3.

· Specialty installation experience and finished-facility protection experience.

· Direct-workforce and supervision plan and key-employee qualifications.

· Equipment, rigging and elevated-access qualifications appropriate to proposed methods.

· Safety record disclosures and safety-planning capability.

· Project-coordination experience and ability to provide bonds and insurance.

· Completed and signed Section 11 below.

Unless separate County forms are provided, prepare the Qualification Statement and Self-Performance Certification in the Bidder’s own format. Address every applicable requirement. Authorize verification of licenses, registration, certifications, references, safety information and employment relationships. Promptly notify the County of a material change before award. Listing a construction subcontractor is inconsistent with the material self-performance requirement.

11. BIDDER CERTIFICATION

I certify under penalty of perjury that the information submitted under this Attachment is accurate and complete; that the Bidder will self-perform the Work as required; and that the County may verify the submitted qualification information with licensing, registration, safety, employment and project-reference sources.
