About this file

This document contains a past and present performance questionnaire template for evaluating contractors on federal projects. The questionnaire requests information on the contractor's performance on up to three relevant past projects within the last five years, including their role and a description of work performed. References are asked to rate the contractor's performance on a scale of 1 to 6 across 20 criteria, such as managing personnel, quality control, scheduling, and responsiveness. References must provide narrative explanations for any ratings of 1 or 2. Additionally, references are asked yes/no questions regarding issues with the contractor, and whether they would award the contractor another contract. General information on the contractor and reference project is entered in blue shaded boxes. The completed questionnaire is returned directly to the contracting office listed in the associated federal solicitation for a roof repair project at a Department of Veterans Affairs medical center.
