VENDOR INSURANCE REQUIREMENTS

All vendors and contractors that service the building on-site at 675 N. Randolph St., Arlington, VA 22203 must provide a certificate of insurance. Please ensure that a current certificate of insurance is forwarded to our office as soon as possible. The certificate must be on an ACORD 25 form and must include the following limits of coverage:

• Commercial General Liability (Including Premises, Operations, & Contractual Liability) o Per Occurrence Limit $1,000,000.00 o Personal & Advertising Injury Limit $1,000,000.00 o General Aggregate $2,000,000.00 o Product/Comp Ops Aggregate $2,000,000.00

• Professional Liability (If applicable) o General Aggregate $2,000,000.00 o Self-Insured Retention $1,000,000.00

• Architects & Engineers (If applicable) $3,000,000.00

• Business Automobile Liability $1,000,000.00

• Umbrella/Excess Liability $5,000,000.00

• Workman’s Compensation VA Statutory o Bodily Injury by Accident (per accident) $1,000,000.00 o Bodily Injury by Disease (per employee) $1,000,000.00

The following MUST be named as the certificate holder as their interest may appear:

The following is the information as it is to appear on the Certificate in the “Description of Operations” block as additional insured:

• Kodiak Realty Services, LLC (Manager)

The certificate must identify any deductibles or self-insured retentions; if there are none, please state this.

The cancellation clause of the certificate must read as follows:

Should any of above described policies be canceled, not renewed, changed materially in amount of coverage, or changed in insured form, the insured company will give 30 days prior written notice to the Certificate Holder.
