The title sheet must include project name, contractor information, contract number, Task Order number, project number, plan date, and version status, with signature by the Corporate Quality Management representative.
The work plan must contain nine sections addressing: project introduction with contract details and site description; project management structure and personnel responsibilities including Program Manager, Project Manager, Superintendent, Health and Safety Officer, and Contractor Quality Control Officer; physical security and visitor control procedures; detailed work description with sequence, equipment, and permits; hazardous material removal procedures if applicable; utility disconnect and cap coordination; demolition means and methods with waste management plans; site restoration specifications if applicable; and references. The document must include nine appendices in specified order: Statement of Work and Modifications, Site Maps, Quality Control Plan with personnel resumes, Environmental Protection Plan (if applicable), Storm Water Pollution Prevention Plan (if applicable), Project Schedule, Accident Prevention Plan, and Existing Condition Photographs. Sections not applicable to a specific project must be marked "Not required for this project" while maintaining placeholder space.
