About this file

This is a Qualification and Certification attachment template for RFP 75A50226R00024, which is part of the Department of Health and Human Services Administration for Strategic Preparedness and Response (ASPR) solicitation for Emergency Response Wraparound Services. The document instructs offerors to complete four sections: Background (providing official company name, address, DUNS number, and primary/alternate points of contact), Qualifications (submitting verification documents including FPDS-NG reports and past performance information as specified in Section M.1 of the RFP), Certifications (accepting all terms and conditions while certifying that cost/price proposals reflect estimates or actual costs), and Authorized Signature (requiring a signed certification by an authorized representative).

The template does not specify substantive requirements for the services themselves but rather serves as a qualification submission format. Offerors must ensure their official company information matches their SAM.gov DUNS registration and provide authorized representatives capable of committing the company contractually. The document includes standard FAR certifications regarding record examination and warnings about falsification of proposal documents. Proposal submission was due September 24 at 5PM eastern time, with questions due September 22 at 12PM eastern time.
