Claims System Modernization - Mandatory Criteria Certification Form
This is a Mandatory Criteria Certification Form issued by the State of Florida Department of Financial Services for the Claims System Modernization Invitation to Negotiate (ITN) No. 2526-01. The form requires respondents to certify compliance with five mandatory criteria questions regarding authorization to submit responses, conflict of interest disclosures, registration with the Florida Department of State, and contract termination history under section 448.095(2)(c), F.S. Respondents must also complete the Form PUR 7801 (Vendor Certification Form), which addresses compliance with Florida Statutes sections 287.135, 287.133, 287.134, 287.137, 287.1351, 287.138, 908.111, 448.095, 286.101, and 787.06. Failure to provide complete certification for each question or the PUR 7801 will result in the respondent being deemed non-responsive. Annual resubmissions of attestation forms will not require written contract amendments. The form includes supplementary attestation requirements via Form PUR 1355 (Foreign Country of Concern Attestation), Form PUR 1808 (Common Carrier or Contracted Carrier Attestation), and Form PUR 2024 (coercion for labor and forced labor certifications).
The certification form requires respondents to attest to compliance with multiple Florida statutory requirements, including non-participation in boycotts of Israel, non-listing on state scrutinized company lists, absence from suspended or convicted vendor lists, E-Verify registration for newly hired employees, compliance with disclosure requirements, and abstinence from coercion for labor or services. If the contract value exceeds $1 million—which applies to this Claims System Modernization ITN with an estimated cost of $8,225,658—the vendor must certify non-involvement with Scrutinized Companies with Activities in Sudan or Iran Terrorism Sectors, absence of business operations in Cuba or Syria, and non-listing on the State Board of Administration's Scrutinized List of Prohibited Companies. All certifications are submitted under penalty of perjury and must be signed by an authorized representative, with the Department reserving the right to deem responses non-responsive or terminate contracts if any information is discovered to be false.
