About this file
This is a Past Performance Questionnaire template used by the Federal Bureau of Investigation to evaluate contractor performance on past relevant contracts. The questionnaire is intended for references to complete and submit regarding an offeror's prior experience with similar work, specifically in support of the FBI's procurement of ten Crime Scene Management Vehicles (Solicitation Number 15F06726Q0000291). References must submit completed forms directly to Contracting Specialist Jamie Compton at jlmelzer@fbi.gov and Contracting Officer Maureen Murdock at mmurdock@fbi.gov, with the email subject line formatted to include the solicitation number and evaluated company name.
The questionnaire requires references to provide detailed project identification information including the contractor name, contract or order number, project title, period of performance, and reference contact details. References are then asked to rate the contractor's performance across six evaluation criteria—Quality of Service, Schedule, Cost Control, Business Relations, Management of Key Personnel, and Utilization of Small Business—using a five-point adjectival rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory, with a Not Applicable option for Cost Control). Each rating category includes detailed definitions to guide consistent evaluation. At minimum, any Marginal or Unsatisfactory ratings must be supported by additional substantiating comments. The questionnaire concludes with an open-ended question asking whether the reference would have reservations about soliciting the contractor for future critical and demanding programs.
