About this file

This is a Past Performance Questionnaire template for the Bureau of Engraving and Printing (BEP) Supervisory Control and Data Acquisition System (SCADA) Maintenance Service solicitation (2031ZA26Q00009). Offerors must submit completed questionnaires from three assessors/references who have provided relevant services within the past five years, where relevant is defined as similar in size, scope, and complexity to BEP's requirements. The offeror completes Section II of the questionnaire, which assessors must validate and confirm before completing Sections III and V. Assessors must email completed questionnaires directly to kaiti.williams@bep.gov no later than 10:00 AM Eastern Standard Time on March 9, 2026, not the offeror.

The questionnaire requires assessors to evaluate contractor performance across five key areas: Business Relations (responsiveness to customer needs and personnel qualifications), Timeliness (task completion within requested timeframes and emergency response times), Quality (overall performance assessment, specification compliance, and workmanship standards), and Cost (ability to accurately estimate costs, meet proposed pricing, and submit timely invoices). Assessors must rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) and can serve in various roles including Procuring Contracting Officer, Administrative Contracting Officer, Contract Specialist, Contracting Officer's Representative, or Technical Project Lead. The questionnaire also includes a yes/no question regarding whether the assessor would award the contractor another contract, with space for explanatory comments if the answer is no.
