About this file

This is a Past Performance Questionnaire template required by the Bureau of Engraving and Printing (BEP) for solicitation number 2031ZA26Q00009 (BEP SCADA System Maintenance Service). Offerors must submit the questionnaire to three assessors/references who provided similar services within the past five years, where relevance is determined by size, scope, and complexity comparable to BEP's Supervisory Control and Data Acquisition (SCADA) System Maintenance Service requirement. The offeror completes Section II with contract information before submitting to assessors, who then validate the information and complete Sections III and V independently. Completed questionnaires must be submitted by the assessor directly to kaiti.williams@bep.gov no later than 10:00 AM Eastern Time on April 16, 2026.

The questionnaire requires assessors to evaluate contractor performance across five key areas: Business Relations (responsiveness and personnel qualification), Timeliness (task completion and milestone adherence), Quality (overall performance assessment and conformance to requirements), Cost (estimation accuracy and invoicing timeliness), and an overall contract award recommendation. Assessors rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) and must identify their role in contract administration (such as Contracting Officer, ACO, COR, or Project Manager). The questionnaire template includes fields for contract details (contract number, type, value, period of performance) and contracting role designation (prime or subcontractor), along with termination history documentation.
