About this file

This document is a Past Performance Questionnaire (PPQ) for a Veterans Health Administration (VHA) construction project to renovate multiple buildings for mental health services. The solicitation (36C26025R0074) is a full and open, unrestricted construction project with an estimated value between $5M and $10M, specifically targeting renovations in Buildings 217A and 218A. The PPQ is designed to evaluate a contractor's past performance across seven key areas: quality, schedule/timeliness, communication, management/personnel, cost/financial management, subcontract management, and overall performance. The form includes detailed rating scales from "Exceptional" to "Unsatisfactory" and provides space for clients to provide comprehensive feedback on a contractor's previous work, which will be used to assess their capability for the current mental health services renovation project.
