About this file

This is a Hazardous Energy Control Contractor Compliance Certification Document required for contractors performing work at Portsmouth Naval Shipyard (NAVSHIPYD PTSMH). The document, Revision D dated February 2, 2022, serves as a certification that contractors understand and agree to comply with hazardous energy control (HEC) requirements before commencing work at the facility.

The document establishes that contractors are responsible for ensuring employee safety during any tasks or work involving hazardous energy control, with compliance required under 29 CFR Part 1915.89 (effective October 31, 2011) and the Naval Sea Systems Command (NAVSEA) Occupational Safety, Health and Environmental Corporate Manual (OSHECM) Chapter 250, including Lockout/Tags-Plus (LOTP) procedures. Upon arrival at the shipyard, contractors must in-brief with the assigned Lockout/Tags-Plus Coordinator (LOTC) to determine if coordination is needed; once the in-brief is complete and the LOTC provides authorization, the contractor may commence work. The document requires completion of fields including purchase order number, job location, company name, equipment status, and contractor representative information. A LOTP Serial Number is required only when coordination is deemed necessary. The Portsmouth Naval Shipyard explicitly disclaims liability for contractor non-compliance, which may result in serious injury or death to contractor employees. Completed certifications must be emailed to the LOTP Chair (Stephen.e.toth2.civ@us.navy.mil) and Shop 06 EMMS Supervisor (Brian.s.cribby.civ@us.navy.mil).
