CONSTRUCTION STATEMENT OF WORK FOR

Project Name: AMLK B19 Replace HVAC Phase-II and Replace HVAC HP3 Project Numbers: 663A4-23-203 and 663A4-24-203

Place of Performance: American Lake Campus

9600 Veterans Drive Southwest Lakewood, WA 98439

Part 1: Project Scope of Work Overview:

The Contractor will provide all labor, material, and equipment to perform a one for one replacement which must be new (or better than) the current existing unitary HVAC systems (1-HP2A, 1-HP2B, and 1- HP3) and all supporting apparatus vicinity Building 19 in accordance with the published contract design and specifications. The Government need for the new HVAC systems is that they are to be safe, reliant, energy efficient, fully digitized and automation controls consistent with gateway and BACnet interface for trends and networking. The new HVAC systems are to be installed and within tolerance delineated in the Manufacture’s Installation instructions. The new HVAC systems will be submitted in accordance with the contract submittal process and approved by the Government prior to the Contractor proceeding with any part of the scope of work.

Phase-II HVAC systems provide air conditioning to five (5) HVAC zones which provide heating and cooling to over 2,500sqft of interior building space that is consistent with staff offices and cubicles. The existing HVAC condenser units and associated mechanical equipment, once removed, becomes the property of the Contractor. Construction debris must be removed and disposed of in accordance with the Department of Veteran Affairs Infection Control Procedures, Federal and State Environmental Protection Procedures and the published construction waste management policy for contractor work on a VA Campus. The period of performance for this scope of work is two-hundred and seventy-two calendar days from the notice to proceed. All work is to be performed during normal operational business hours;

work performed outside daily operating hours must be approved by the Contracting Officer 14-days prior to the intended scheduled off-hour work commencing.

CLIN0001 (663A4-23-203): Base contract work perform one for one replacement of existing HVAC Outdoor Units (1- HP2A) and (1-HP2B) with new, or better, ODUs to include supporting component systems and applicable apparatuses.

CLIN0002 (663A4-24-203): Base contract work perform one for one replacement of Decentralized Unitary HVAC Outdoor Unit (ODU) designated Heat Pump (1-HP3) to include supporting mechanical units, component systems, and applicable apparatuses.

Part 2: Period of Performance (POP) and Project Construction Cost Magnitude:

1. Complete all work including submittals, reviews and quality control within 279 calendar days from

Notice to Proceed (NTP) as shown below. NTP may be delayed for outside work until weather permits.

Accommodate the Period of Performance by including accelerated equipment delivery costs as well as adjusting crew sizes.

Part 3: Construction Progress Meetings:

1. The Contractor’s Project Manager and Site Superintendent shall provide bi-weekly (every other week) telephone progress meetings with the COR during the survey and submittal portion of the period of performance.

a. The Contractor’s Construction Quality Control (CQC) Manager is required to attend these meetings. The CQC will update the GOV on the internal review process of sub-contractor submittals and provide an updated submittal register delineating current status of all pre-construction and material submittals.

b. The Contractor’s Construction Safety Officer is not required to attend.

c. Subcontractors are not required to attend.

d. Include minutes of the meeting and issue not later than three days after the meeting. Both PDF and Word/Excel format

e. Construction progress meetings minutes shall contain list of attendees, list any issues or concerns, indicate days remaining per the contract, detail any safety incidents, RFI log, Shop Drawing Log, detailed 2 weeks look ahead and shutdown schedule.

2. The Contractor’s Project Manager and Site Superintendent shall provide weekly onsite progress meetings with the COR for the construction portion of the period of performance (from onsite mobilization to punch list completion).

a. Total labor hours performed on the project:

i. By crew and by day for the current week of work

ii. Cumulative Total work hours performed from the start of construction.

b. Highlight upcoming Definable Features of Work (DFOW) and associated Preparatory

Meeting date, time and contractor performing the work.

c. Describe in detail Follow-Up work of current work being performed and or completed including testing and closeout documents.

i. Testing Log

d. Identify any rework needing to be accomplished.

i. Rework Listing

e. Provide review of current submittal register and statuses of pending or open submittals.

f. The Contractor’s Construction Safety Officer is required to attend.

g. Subcontractors Project managers are required to attend.

h. Include minutes of the meeting and issue not later than three days after the meeting. Both PDF and Word/Excel format

i. Construction progress meetings minutes shall contain list of attendees, list any issues or concerns, indicate days remaining per the contract, detail any safety incidents, RFI log, Shop Drawing Log, detailed 2 weeks look ahead and shutdown schedule.

3. At 25%, 50%, 75% and 95% completion, coordinate with the COR for walk through inspection with the VA Shops. The Contractor shall have the Foreman from all trades attend. The VA Shops that will attend is the following: Electrical, Electronics, Mechanical, Operations, and Structural Maintenance Shops. The purpose of the meeting / walkthrough is the VA Shops Leads talk with the respective Contractor Trade Forman / Superintendent

Daily Report:

1. The Contractor shall provide a Daily Report. The Daily Report shall contain list any issues or concerns, indicate days remaining per the contract, detail any safety incidents, detail daily construction safety inspections including items checked and corrective actions taken, and include relevant construction progress photo(s). Email the Daily Report to the COR not later than 9 am the next working day. Contractor is not required to submit a Daily Report for Saturdays, Sundays, or federal holidays where no work takes place.

Part 4: Submittals and Milestone Deliverables:

1. KICK-OFF MEETING with DOCUMENTATION

a. The Contractor’s Project Manager and Site Superintendent shall attend a Preconstruction Safety Meeting, to discuss the project details.

b. Submit: Provide a FULLY LOADED COST SCHEDULE Not less than one item per subcontractor. In addition, include any expected system shutdowns and off hours work.

c. Submit: LIST OF ALL CONTRACTORS AND SUBCONTRACTORS: company names, office address and main point of contact name with telephone number

d. Submit: CONSTRUCTION SAFETY PRECAUTIONS with project specific procedures, processes, analysis, and details including but not limited to:

i. CONSTRUCTION SAFETY PLAN

ii. ACCIDENT PREVENTION PLAN (APP) project specific

iii. ACTIVITY HAZARD ANALYSIS (AHA) project specific

e. Submit: CONSTRUCTION INFECTION CONTROL / DUST CONTROL PRECAUTIONS including:

i. Provide project specific details for construction infection control / dust control precautions for review and approval of the VA.

ii. In preparation of this item: Coordinate and meet with the COR to obtain approval of the CONSTRUCTION INFECTION CONTROL / DUST CONTROL PRECAUTIONS by VA Infection Control. Include one on-site meeting with the VA COR in perpetration and one on-site meeting with the VA COR and the VA Infection Control Staff.

f. Submit: SHOP DRAWINGS AND PRODUCT LOG:

i. List all shop drawings and products in the submittal log.

g. Submit: Scanned copy of all workers OSHA Construction Safety Training certificates.

2. SHOP DRAWINGS AND PRODUCT SUBMITTALS

a. The Contractor shall provide all shop drawings and product submittals by email to the COR or through a submittal exchange.

b. Allow 14 calendar days for VA review, unless otherwise stated in the Period of Performance

Chart.

3. MOBILIZATION ON SITE INCLUDING PERFORMANCE OF THE WORK

a. Contractor shall not mobilize on site until all KICK-OFF MEETING AND DOCUMENTATION and SHOP DRAWINGS AND PRODUCT SUBMITTALS are reviewed and accepted by the VA.

b. Approval to mobilize shall be in writing by the COR.

c. Post and maintain emergency contact information, VA Infection Control Risk Assessment, and other information required by the COR, on the wall at all entry points to the worksite.

4. ACCEPTANCE AND CLOSE OUT

a. Submit a written statement from the Contractor attesting that the Work is Substantial Complete:

i. The final construction inspection and the related work corrections shall be completed before the end of the period of performance. Prior to expected Substantial Completion, at 75% construction complete, notify the CO and COR.

Provide the following:

1. This notification will allow the COR to coordinate final construction completion inspection.

2. This notification will allow the COR to coordinate with Operations and Maintenance Staff’s schedules to accommodate Demonstration and System Training.

3. With this notification, the Contractor shall provide the COR a list of items that are incomplete / incorrect. This list of incomplete / incorrect items will allow the Project Team to understand what work remains so that construction inspection findings are not incorrectly identified as a deficiency.

ii. At Substantial Completion, the Contractor shall notify the CO and COR in writing that work is complete, and the Contractor has completed their quality control measures.

b. Demonstration that the work is complete:

i. The Contractor shall demonstrate the operation of the system(s).

ii. Provide factory startup and certification of all systems.

iii. Newly installed piping and distribution system components shall be flushed and disinfected prior to being placed in service. Include documenting completion.

iv. Schedule the demonstration with the COR, 21 days in advance so that the VA

Operations and Maintenance staff and the Designer of Record can attend.

v. One demonstration of the correct operation of the system is required. Coordinate for during the day, however if staff and patients are impacted by the demonstration then it shall be scheduled at night or on the weekend.

vi. If the demonstration does not show expected operation, the Contractor shall make required system adjustments and reschedule and retest.

vii. This demonstration is not to be combined with the System Training functional performance testing.

c. Closeout Records: Provide one (1) full-size plan printed set and two (2) compact discs with electronic files of the following:

i. Final drawings and specifications complete, identified and marked as Issued for Construction, and the licensed principal design discipline lead responsible for each sheet.

ii. Plan as built record drawings of the installed condition shall be CAD drawn. Turn over the CAD files (do not bind xrefs), PDF files and the red line field notes to the VA Approved shop drawings.

iii. Operations and Maintenance Manuals

iv. Spare parts listing

Part 5: Applicable Performance Standards

1. All work shall comply with

a. All VA Policies including and not limited to: VA Construction Safety and VA Infection Control Policies.

b. Codes, Standards and Executive Orders (Topic 1) found http://www.cfm.va.gov/til/cPro.asp

2. All persons including subcontractors shall have completed the 30 Hour OSHA Construction Safety

Training program or 10 Hour OSHA Construction safety Training program per their designed role. (see bid specification 01 35 26.1.8.A-D.)

3. The Prime Contractor shall have a supervisor (Foreman, Superintendent, or Project Manager) on site always when any worker(s) or sub-contractors are present.

4. The Contractor shall provide mitigating activities as outlined in the CONSTRUCTION SAFETY PRECAUTIONS and CONSTRUCTION INFECTION CONTROL RISK ASSESSMENT / DUST CONTROL PRECAUTIONS including but not less than the following:

a. Fire extinguishers, pedestal mounted in the area of work.

b. Walk off dust mats, inside and outside all entry / exit points to the workspace, changed not less than daily or more as required by the construction or COR.

c. Wash / wipe all tools and equipment before entering the surgical suite.

d. Do not allow corrugated carboard into the surgical suite.

e. No trash, used packaging, or construction spoils shall be stored on site. Remove each day using covered gondolas.

5. All work will be provided with one-year parts and labor warranty from date of acceptance by VA.

Part 6: VA Puget Sound Health Care System Additional Requirements

1. Project number and the contract number in the subject line of all emails.

2. Contractor shall comply with the VAPSHCS Smoking Policy.

3. Normal construction work hours are 7:00AM to 4:00PM Monday through Friday, unless other specified in the SOW.

a. Normal work hours can be modified only by written approval of the COR.

b. No work by the Contractor on Federal Holidays, and no work on the weekend when the

Federal Holiday is on the respective Monday or Friday.

c. Work in stairs, or interior / exterior public areas shall be allowed between 6:00 PM through midnight, concluding by 6:00 AM on the following day.

d. Any work including, and not limited to: Noise, vibration, dust, odors, core drilling, hammer-drilling, saw-cutting, equipment moving in public corridors, soil compaction, or similar noise-producing activities shall be performed between 6:00 PM through midnight, concluding by 6 AM on the following day.

e. Crane lifts, any work that closes drive isles, closes building entries or closes public corridors shall be during low patient time and performed Sundays.

f. The Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) http://www.cfm.va.gov/til/cPro.asp which is located in Patient Admitting for any work between 6:00 PM through midnight to 6:00 AM or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD.

4. All building systems will be maintained in full operation at all times.

a. Request system shutdowns in writing not less than 14 calendar days in advance.

b. Shutdown work shall be during low patient demand times, and performed between

Saturday 6 PM through midnight, concluding by 6 PM on the following Sunday.

5. Process for excavation is as follows:

a. Identify the areas where excavation is required, and mark for COR review.

b. Contractor shall verify buried utilities with VA record documents and municipal record documents.

c. Contactor shall contact all utilities for locator services in the area of work.

d. There is existing non-utility owned buried services and the Contractor shall provide locator services such as ground-penetrating radar and radio frequency techniques.

e. Before excavation the contractor core paved areas to verify concrete/pavement depth.

Coordinate the location of the holes with the COR before drilling.

f. Hand dig all non-paved areas prior to excavation

6. Deliveries, Parking, Elevators:

a. Deliveries only at the Loading Dock at Building 143, and coordinated with the COR. At time of delivery: provide vehicle information to the VA Police and remove the vehicle as soon as delivery is complete. Move materials in public corridors during low patient time and performed between 6 pm through midnight concluding by 6 AM on the following day.

b. No on-site parking or parking in the VA garage or VA parking lot. The existing VA parking is at capacity and reserved for patients and staff.

7. Contractor shall provide construction dumpsters with a surrounding safety fence. Pavement to be protected from damage of loading and unloading the dumpster and dumpster weight. Located on the Loading Dock at Building 143. Provide recycling and receipts from the salvage / deposal service in compliance with VA’s GEMS Policy.

8. On-site office or on-site storage / office container shall be consolidated within approved contractor fence, co-located with contractor dumpster and temporary toilet. Coordinate with COR for staging within the area of work.

9. No music radios. Two-way communications radios are allowed where broadcast frequency is approved by COR.

10. No smoking on VA campus and associated facilities.

11. All work in the crawl spaces shall be planned and carried out as OSHA Permit Required Confined

Space.

12. Immediately stop work and notify the COR if asbestos is noticed or suspected in the area of work.

13. Before starting work the Contractor shall give 30 days’ notice to the COR so that security arrangements can be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section.

14. No photography of VA premises is allowed without written permission of the COR.

15. VA reserves the right to close or shut down the project site and ask Contractor’s employees to leave the premises in the event of an emergency, or operational requirement. The Contractor may return to the site only with the written approval of the COR.

16. Compatible System procurements shall be as follows:

a. Locks and Keys: Door hardware shall be compatible with Best® 7-pin, F keyway, interchangeable core. Final pinout and keying are provided by the VA. Provide and turn over new blank cores to VA not less than 21 days in advance of construction completion.

The final pinout and key blanks shall be provided by VA.
