This document contains a past performance questionnaire template for evaluation of contractor references on a federal solicitation. The solicitation number is 1305M2-21-Q-NFFK-0097 from the Department of Commerce National Oceanic and Atmospheric Administration for a vessel monitoring system. The past performance questionnaire is to be completed by contractor references for relevant projects of similar size, scope and complexity performed within the last three years. References will provide ratings and comments on six criteria: quality of service, schedule, cost control, business relations, management of key personnel, and management of teams/subcontractors. Ratings use an adjectival scale of exceptional, very good, satisfactory, marginal and unsatisfactory. The completed questionnaires must be submitted directly from the reference to Shawn.Griscom@noaa.gov by the closing date of the solicitation.
