The performance evaluation covers 22 specific areas including management quality, personnel capabilities, workload performance, timeliness of deliverables, security compliance, quality control procedures, problem identification and resolution, responsiveness to changes, project supervision, and subcontractor payment practices. Additional questions address whether the contractor received corrective action notices, whether the respondent would award another contract to the contractor, CPARS ratings, and contractor communication regarding issues. Respondents must provide supporting information for each rating and may attach additional pages as needed.

Past Performance Questionnaire
The 1st Special Operations Contracting Squadron at Hurlburt Field, FL is competitively selecting a source for Integrated Defense Base Security Systems.
One of the considerations in the evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 1:00 PM CST, 12 February 2026. This schedule will allow sufficient time to analyze the data.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to kodei.calvin@us.af.mil and justin.carroll.4@us.af.mil.
