About this file

This is a Past Performance Questionnaire template designed for the 48th Contracting Squadron at RAF Lakenheath to evaluate offerors' qualifications for the UK-Wide Integrated Waste Management Services contract. The questionnaire is completed by previous customers or contracting officers who have direct experience with prospective contractors and serves as part of the official source selection record.

The questionnaire requests detailed information organized into four sections: contractor identification (company name, CAGE code, contract number, contract type, program title, period of performance, and contract value); customer or agency identification; respondent identification; and performance information. The performance assessment section uses a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) across 26 evaluation criteria, including contractor management capabilities, quality control procedures, personnel adequacy, problem resolution, responsiveness to changes, and overall performance. Additionally, the questionnaire includes targeted questions assessing whether the contractor has managed multiple regions simultaneously, provided services across multiple counties, managed at least 290 Front End Loaders and 1.2 million liters of liquid waste, performed full refuse maintenance services, managed subcontractors without service gaps, achieved minimum annual contract value of £4,000,000 ($5,380,000), and experienced employee retention issues. Respondents are instructed to provide supporting information for each rating and submit completed forms electronically to 48CONS.PKB.Refuse@us.af.mil by a deadline to be determined.

For Official Use Only When completed, this document is Source Selection Information IAW FAR 2.101 and 3.104.

Attachment 3: Past Performance Questionnaire The 48th Contracting Squadron, Royal Air Force (RAF) Lakenheath, United Kingdom (UK) is in the process of competitively selecting a source for UK Wide Integrated Waste Management Servies (IWMS).

The Contractor shall provide all personnel, equipment, containers, tools, materials, vehicles, supervision, and other items and services necessary to perform the IWMS described in this performance work statement (PWS) at U.S. Visiting Forces (USVF) locations across the UK. IWMS consists of solid (including recycling), non-hazardous and hazardous liquid waste collections and removal. As of the date of this PWS, USVF locations include RAF Lakenheath, Feltwell, Mildenhall, Alconbury, Blenheim Crescent, Det 6, Croughton, Fairford, Molesworth and Welford. The Contractor shall perform to the standards in the contract, as well as all applicable United Kingdom laws and regulations, international agreements, Status of Forces Agreements (SOFA) and appropriate Final Governing Standards (FGS) or in their absence, the Overseas Environmental Baseline Guidance Document (OEBGD).

One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response TO BE DETERMINED. This schedule will allow us sufficient time to analyze the data prior to the start of any negotiations.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to 48CONS.PKB.Refuse@us.af.mil. Your completed questionnaire will become a part of the official source selection records.

Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
