About this file

This document is a McConnell Air Force Base (MAFB) Short Term Pass Request form for sponsoring visitors to the military installation.

The request is limited to five guests per day and is designated as Controlled Unclassified Information (CUI). All fields in the form are mandatory. Requests must be submitted at least 24 hours prior to the visitor's arrival; same-day requests will not be processed. The sponsor must ensure visitors have current driver's licenses available for review. Contractors and vendors using company vehicles must have their vehicles inspected during weekday hours between 0600 and 1400 and must use the WEST/CVI gate. Commercial vehicles destined for Mayroad Military housing will be inspected and processed at the Visitor Center. The pass request is limited to a maximum of ten days. The installation prohibits weapons, fireworks, explosives, talking, and texting while driving (except on hands-free devices). Upon completion, the requester must save and encrypt the form and email it to the appropriate office: 22sfs.s5p@us.af.mil for guests/visitors or 22sfs.s5pv.officialpasses@us.af.mil for contractors/vendors. Additional information is available from the Visitor Center at 759-4712 or 4713, or the Contractor Badge Office at 759-1781. A single sponsor may only sponsor a maximum of five persons per day and cannot sponsor Special Events or non-U.S. nationals.
