Contractors and subcontractors are required to meet all requirements listed in the current Grissom NPDES permit. Construction Stormwater General Permit (CSGP) will be required for all construction activity (which includes clearing, grading, excavation and other land-disturbing activities) that results in the disturbance of one acre or more of total land area. This permit serves as a National Pollution Discharge Elimination System (NPDES) general permit and is issued to be effective for a term of five years. Contractors will be required to submit an online application to obtain a CSGP by submitting a construction/stormwater pollution prevention plan (SWP3) and a notice of intent (NOI) to Indian Department of Environmental Management (IDEM). Reviewing authority (IDEM) has up to 28 days from the date of submittal to review the SWP3 and if changes are required, resubmittal of the SWP3 changes will allow approving authority an additional 28 days to review. Contractor NOI letter can be submitted to IDEM after SWP3 is approved. NOI letters must include proof of publication notification in local newspaper (Peru Tribune) for one day, proof of SWP3 approval, and $100 fee. The NOI must be submitted to IDEM a minimum of 48 hours prior to the initiation of land disturbing activities. Contractor will authorize the CEV program manager online viewing of the CSGP through the IDEM ePortal. Monitoring and Project Management requirements will be conducted 24 hours prior to a qualifying precipitation event (equal to or greater than .5 inch or rainfall within a 24-hour period) or once a week if no rain event occurs in accordance with CSGP. Report and documentation requirements can be found within the CSGP guidelines in section 3.6 and 3.7. Upon project completion, CEV program manager must authorize permittee to submit a notice of termination (NOT). NOT will not be granted unless there is 80% vegetation growth (Grissom Blend seeding recommended) and land disturbance have ceased. CEV program manager will be notified when NOT has been submitted and approved by IDEM through the ePortal by the submitter. A CSGP/NPDES checklist can be obtained upon request by the CEV program manager. All stormwater inspections will be submitted to CEV quarterly, annually and at project completion.

Contractor is to report the spill (per SPCC/FRP), same day, to CEV and any other pertinent party. A complete list of spills during the contract will be provided quarterly and at project completion.

Asbestos:

Asbestos maybe present on GARB. Most buildings were built prior to 1970 and have been renovated several times. Due to the destructive nature of asbestos sampling and the potential for ACM to be covered or hidden, there might still be unknown locations of asbestos present on Grissom. The base used to have a steam heat system, which most of the underground pipes are still in place. The wrapping on the piping may contain asbestos. All personnel (government & contract) will comply with the asbestos management plan. Contractors are required to provide an asbestos inspection performed by a licensed asbestos inspector. A copy of the asbestos inspector’s license is to be provided and received by CEV prior to the inspection. Contractor is to provide a map of all sampling locations, sampling numbers, chain of custody forms, sample results and any other asbestos related information (such as asbestos determinations and recommendations) to CEV before any demolition work begins. Contractor is responsible for any permits required for asbestos work (removal, disposal, air monitoring, etc.). If the requirements for IDEM Notification of Demolition and Renovation Operations (State Form 44593) are met, the contractor is responsible for completing notification (to include fees) and meet all requirements.

IDEM Notification of Demolition and Renovation Operation Requirements:

· All demolitions

· All renovations disturbing RACM of 260 linear feet on pipes, 160 sq ft on other facility components or 35 cubic feet off facility components

· Reportable changes

· Increase or decrease in RACM by 20%

· Start dates

· Demolition contractor

· Asbestos contractor

· The intended waste disposal site If notification to IDEM is required, contractor must wait 10 working days from the form’s post-mark or delivery date to:

· Begin demolition

· Begin a renovation that involves at least 260 linear feet, 160 square feet, or 35 cubic feet of RACMs and is not necessitated by a sudden, unexpected event.

If asbestos removal is required, a copy of all asbestos personnel licenses must be submitted to CEV prior to any asbestos abatement work commencing. The Asbestos Program Officer (CEV) will be notified by the abatement contractor when the asbestos material has been properly containerized and is ready for transport. The contractor will provide CEV copies of any transportation and disposal records (such as weights, disposal location, manifests, etc). If material is disposed on a manifest with Grissom ARB’s EPA ID number, CEV is required to sign the manifest.

Lead-Based Paint:

Grissom ARB currently has no residential housing, target or child occupied facilities as defined by HUD; therefore, all buildings at Grissom ARB are designated as non-priority buildings. All work will be in accordance with Grissom ARB’s Lead Based Paint Management Plan. Contractors must comply with all regulations governing LBP projects (including regulations regarding occupational safety and health, control of hazardous emissions, notification requirements, proper labeling and posting, and disposal).

Many of Grissom ARB’s buildings were built prior to 1980, though most have been remodeled, there is a potential for lead-based paint (LBP) to be present. If testing is conducted, a copy of all testing results will reviewed by CEV prior to any lead removal activities.

If abatement activities are to occur, Contractors are to provide licenses for all individuals working with LBP (including a statement for the supervisor which indicates this person will provide daily supervision of the project), a copy of the contractor’s lead abatement license, a copy of the contractor’s certificate of liability and lead/hazardous materials specific insurance, to CEV prior to any work being started.

LBP construction debris from non-residential structures may be managed as C/D wastes as defined by 329 IAC Article 10-2-7 - Solid Waste Land Disposal Facilities and disposed in a state permitted MSWLF or C/D facility. It is not necessary to test debris containing LBP prior to disposal at a state permitted C/D site or MSWLF. Metal components covered with LBP may be recycled as scrap metal.

Waste being removed ‘from Grissom ARB on a manifest, with Grissom ARB’s EPA ID is required to be signed by a member of the CEV office.

Solid Waste/ Recycling:

All personnel (government & contractor) are to follow Integrated Solid Waste Management Plan (ISWMP) for recycling/disposal of all solid waste. Design/construction will maximize the use of materials with recycled content. Contractors will maximize the diversion of construction materials disposal in the landfill (metals, cardboard, plastics, etc.). Construction & demolition (C&D) debris generated from specific construction, renovation, and maintenance projects is the responsibility of the contractor. Contractors will meet or exceed the current Air Force goal for C&D diversion (currently at 60%). Contractor is responsible for proper disposal of all items. Contractors are responsible for any permits and fees required for solid waste disposal/recycling. Contractors will provide quarterly and annual (FY calendar), and project completion reports to CEV for materials disposed and diverted. Reports will contain, at a minimum, material disposed/recycled, pounds/tons of material recycled/disposed, location material is recycled/disposed (landfill or company), date material left the site, and any other pertinent information. Contractor shall provide, with reports, weight tickets for material recycled/disposed. Contractors may contact current QRP manager for help, as some items might be able to be recycled locally.

Fill Material Any materials being brought onto the installation to be used as fill material must be free of contamination. The fill material will be sampled with results provided and approved by the CEV office prior to the materials being brought onto the installation. Fill materials will be analyzed for VOCs, SVOCs, BTEX, PFAS/PFOA, and Metals.
