About this file

The document is a detailed General X-Ray Inspection Tests checklist dated 10/28/2024, outlining a comprehensive verification process for X-ray system installations. The 14-point checklist covers critical aspects of X-ray equipment acceptance, including verifying delivery order items, software licenses, documentation, emergency circuits, warning lights, mechanical operations, generator calibrations, radiographic and fluoroscopic functions, and ensuring all installation work is completed according to the Performance Work Statement (PWS). Each verification point emphasizes compliance with federal regulations and manufacturer specifications, focusing on system safety, functionality, and operational integrity.

The checklist requires systematic verification of multiple technical components such as emergency power systems, UPS units, door interlocks, patient tables, imaging receptors, radiation field alignments, image quality, and contrast injector operations. Specific technical checks include generator calibration parameters (kV, mA, time, mAs), exposure reproducibility, tube limit settings, photo-timer operation, and density setting accuracy. The document appears to be an attachment to the VA's solicitation 36H79725R0001 for High Tech Medical Equipment (HTME) Inspection Services, which is a small business set-aside contract for testing medical equipment installations.

1. Verify that all items listed in the VA Delivery Order are present.

2. Verify that all purchased System Applications Software License Keys are permanently installed.

3. Verify that all Service, Maintenance, and Operator Manuals are present for the system and all applicable purchased options.

4. Verify Form 2579 is present.

5. Verify that all System Back-up Software is present and is the same revision as the software installed.

6. Verify proper operation of all Emergency Activation Circuits (System Emergency Power OFF (EPO), Table and Control Panel shutdown switches, etc.).

7. Where applicable, verify operation of System and/or Control Console UPS unit (s).

8. Verify proper operation of all X-Ray Warning Lights and, Door Interlock Switches (where applicable)

9. Verify proper mechanical and electro-mechanical operation of all appropriate major system components (e.g., patient table, fluoro table tower, overhead ceiling tube support, wall image receptor support unit, etc.).

10. Verify that the Generator Calibrations (kV, mA, time, mAs, exposure reproducibility, tube limit settings, photo-timer operation, density setting accuracy, etc.) meet Federal Regulations and Manufacturer’s Published Specifications.

11. Verify that all Radiographic Functions (automatic collimation, light to radiation field alignment, digital image receptor operation, tube to image receptor alignment, etc.) meet Federal Regulations and Manufacturer’s Published Specifications.

12. Where applicable, verify that all Fluoroscopic Functions (maximum patient entrance dose (high and low level), fluoro image quality (high and low contrast resolution using the manufacturer’s supplied and/or industry accepted imaging phantoms), radiation field sizes, auto brightness control, image artifact evaluation, etc.) meet Federal Regulations and Manufacturer’s Published Specifications.

13. Where applicable, verify that safe operation of the Contrast Injector (after installation) has been confirmed by the unit’s OEM.

14. Where applicable, verify that all work identified in the Performance Work Statement (PWS) for Extended Installation listed in the VA Delivery Order has been completed (i.e., no open Punch List).
