About this file

This document is a Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) for propane service delivery to Peach Springs Health Center in Peach Springs, Arizona.

The PWS establishes a five-year service contract running from July 18, 2026, through July 17, 2031. The contractor must ensure continuous and reliable propane supply to three locations: the Main Clinic (943 Hualapai Way, Account 46074237), Government Quarters 1 (940 Hualapai Way, Account 46074186), and Government Quarters 2 (938 Hualapai Way, Account 46074186). The critical performance standard is zero instances of service interruption or stock-outs at any facility due to contractor action or inaction. The contractor retains full responsibility for logistics, inventory management, delivery scheduling, and routing to meet this standard, with the government exercising outcome-based oversight rather than dictating operational methods.

The QASP sample specifies how the Peach Springs Health Center IHS will monitor contractor compliance with an Acceptable Quality Level (AQL) of 100% compliance with zero deviations. Surveillance methods include monthly visual tank level checks at all three locations, recording customer complaints regarding service loss, and periodic account verification through delivery slip reviews and energy audits conducted by the Contracting Officer Representative or Facility Manager. Any failure to meet the AQL will be documented in a Performance Assessment Report provided to the contractor for corrective action, with performance deficiencies potentially resulting in contract deductions or further remedial actions as specified in contract terms and conditions.
