TECHNICAL DESCRIPTION

A Single Ceiling Mount consists of one Visionary® lighthead (surgical luminaire) suspended from the end of a horizontal arm (extension arm) and vertically articulating arm. The arms are connected to a vertical tube (downtube) secured to the structural ceiling. All this is illustrated in the following figures. The lighthead can be positioned anywhere within the great circle defined by the combined length of the extension arm and articulating arm. Up and down movement is limited by the vertical range of the articulating arm. A Single Ceiling Mounted Visionary® does not have redundant wiring, and, therefore, a single fault in the primary or secondary circuits will cause the lamp to go out. Terminology used herein is the same as contained in the International Standard, IEC 60601-2-41, First Edition 2000-02. The common American term, Surgery Light, is synonymous with the European, Surgical Luminaire.

In similar fashion, the configuration of two lightheads mounted off a single downtube from a fixed point on the ceiling is called a Double Ceiling Mount. The horizontal motion of the lightheads on the Double is restricted because of mutual interference of the extension arms (with the downtube, and each other); however, the output from both lightheads can be focused on the same object within the great circle defined above.

When the two lightheads are mounted and the combined light energy directed at a single work area (surgical field), light intensity is essentially twice that of a single lighthead.

Because a “double” Ceiling Mounted Visionary® has two lightheads and redundant wiring, if one of the circuits opens, only one light goes out; the other remains on, and the total fixture can be classified as a fail safe Surgical Luminaire System (provided the illuminance from a single lighthead is more than 40,000 Lux at one meter). The IEC

60601-2-41 terms the Double Ceiling Mount models as a Surgical Luminaire System, or Major Surgical Luminaire, and has defined it as ‘failsafe’ in that one half the total intensity is remaining under single fault conditions.

Each lighthead contains four special bulb/reflector/filter assemblies. When the lighthead is located about 1-meter above the patient field, slight focus adjustments of the

JAMES A HALEY VETERAN'S HOSPITAL

Burton Medical Products viii synchronized bulb pattern can be made by twisting the handle located asymmetrically off the bottom of the housing. This handle, which can be steam autoclaved, also is used to reposition the lighthead anywhere within its full Range of Motion. To remove the handle for sterilization, the operator must push a release rod, located above the handle flange, with a finger or thumb. To reinsert, all that is necessary is to push the handle over the guide rod until it ‘clicks’ into position. The handle also accepts commercial pre-sterilized covers, so it will not have to be removed for sterilization after each procedure.

Bulb changes are made from the front of the lighthead by removing the front window assembly. A small tool (Phillips screwdriver) is required for this. If the light was in use approximately 40 minutes are required for the light to cool down sufficiently for the windows and bulbs to be touched safely. The non-working bulb is then released from its socket by gently pushing, twisting and pulling it out. Disposal of the light bulb must be in accordance with local regulations.

1. Safety precautions when working with the single or double ceiling mount.

• It is the responsibility of the customer to be in compliance with all applicable building codes and load requirements for this product above the ceiling.

• CAUTION: Before undertaking any work on the ceiling mount, ensure that the main power is off and secured from accidentally being switched on again.

• The ceiling mounts must only be installed by a technical expert (electrical engineer, technician) or a comparably qualified person.

• The 120-volt connection must only be made by an electrical engineer.

2. Safety precautions when working with the articulating arm.

• CAUTION: Before undertaking any work on the articulating arm, ensure that it has been disconnected from the main electrical supply!

Burton Medical Products xii

• Electrical connections, installation, removal and repairs should only be carried out by a qualified electrician.

• Do not remove safety caps (3) Figure 11 and (1) Figure 10 until the equipment is ready to assemble, and always ensure that the electrical supply is switched “OFF” until final assembly is completed.

• The articulating arm is tensioned by a spring. Always raise the arm (6) Figure 11 slowly to its upper limit before assembling or dismantling.

• It is recommend that installation of equipment on the articulating arm be carried out by two people.

3. Safety precautions when working with the lighthead.

• Both models are intended for installation on a 15-ampere branch circuit. All tests were performed on a circuit with that rating.

• When relamping the light, make sure that power to the bulb is off.

• Do not open the top for relamping. Change the bulbs by opening the windows at the bottom of the light.

• Avoid relamping when the fixture is hot. Wait approximately 40-minutes for it to cool.

• Replace bulbs with same type and rating. Do not exceed 35 watts per bulb.

Purchase bulbs from a Burton-authorized distributor.

• Do not touch the bulb’s glass envelope with bare hands, as oil from skin will cause premature failure. Use a tissue or Kimwipe®.

• Do not operate fixture with missing or damaged heat filters (coated glass filters are assembled to inside of windows).

• During normal operation do not touch any optical surfaces (glass filter, bulb, reflector). These surfaces are extremely hot and will burn tissue on contact.

• If a bulb burns out just prior to, or during a procedure, continue to use the fixture until a replacement can be made. But be aware that the added voltage, caused by the non-operating failed bulb, will eventually lead to premature burnout of the remaining bulbs.

• Disconnect power to the lamp circuit at the main breaker before replacing fuses.

Replace only with F3A/250V.

• Do not look directly at the light bulb when it is on.

Burton Medical Products xiii

• Do not operate the equipment if the front window assemblies are not retained securely in their respective openings, or if the wire tethers are defective or missing.

• Under normal conditions of use it is not necessary to open the housing from the top.

Normal maintenance items such as bulb replacements are made from the bottom after removing the front windows.

• Do not use the fixture if, for any reason, it does not appear to work properly.

Reasons can include inadequate light output, poor or loose movement, odd noises while moving the lighthead, apparent separation of mating components, etc. Instead, alert your Maintenance Department, or Burton, of observed deficiencies and have them corrected before returning the light to service.

4. Electromagnetic Compliance (EMC).

• It is important to install and use the equipment in accordance with the instructions, to prevent harmful interference to other devices in the vicinity. If this equipment does cause harmful interference to other devices, which can be determined by turning the equipment off and on, the user is encouraged to try to correct the interference by one or more of the following measures:

o Re-orient or relocate the other device(s).

o Increase the separation between the equipment.

o Use different electrical circuits for the conflicting devices.

o Consult with Burton or your field service technician for help.

IMPORTANT INSTALLATION ADVISORY

To: Installers and End Users of all domestic models of Visionary Surgery Lights

Re: Premature bulb burnout in Visionary

Premature bulb burnout may be the result of incorrect installation of your Visionary Surgery Light due to the applied bulb voltage being higher than 26.5 volts. The following are the steps to have your electrician take to verify that your Visionary Surgery Light was installed properly:

1) Verify your MAINS (Input) voltage.

2) If your MAINS (Input) voltage is in the range of 104V – 116V, then the Red and

Gray transformer secondary wiring (taps) should have been hooked up.

3) If your MAINS (Input) voltage is in the range of 117V – 124V, then the Red and

Yellow transformer secondary wiring (taps) should have been hooked up.

4) If your MAINS (Input) voltage is in the range of 125V – 134V, then the Red and

Orange wiring (taps) should have been hooked up.

This information can be found in the Visionary Installation, Operations and Maintenance Instructions on page 24 and 30 (Figure 17) that were provided with your light. For optimal performance of your light, please consult Figure 17 on page 30 of your Visionary Installation, Operations and Maintenance Instructions.

Sincerely, Wayne Okazaki Quality Assurance Manger

1. UNPACKING

1.1 Unpacking and Inspection

The Visionary® Lighthead, arms, and ceiling mount(s) have been packaged to survive normal rigors of shipment. When unpacking the merchandise be sure to note loss or damage. If there is apparent loss or damage, but minimal damage to the shipping container itself, stop unpacking until you have contacted the carrier. In all cases of loss or damage, follow the instructions printed on the following page and do not throw away any cartons until told by Burton

Medical.

1.2 Special Instructions

Save the original shipping cartons until the fixtures are mounted and operational, and the customer is satisfied with the installation.

Burton Medical Products Corp. 2

Burton Medical Products Corp. 3

2. INSTALLATION NOTES

2.1 Range of Motion

Look at the diagrams showing RANGE OF MOTION (Figures 1 through 4) for the Visionary mounts (Single and Double Ceiling Mounts). It is the responsibility of the purchaser to inform Burton and the end user of installation details, including ceiling height, desired height of lighthead above the floor, location of mains power, etc., prior to purchase so there will be no misunderstandings regarding performance of the installed product. Before commencing work, the contractor/installer should review the RANGE OF MOTION diagrams with the customer and confirm the limits of travel.

2.2 Structural Requirements

The ceiling to which the lights are to be attached must be structurally sound, and suitably reinforced in order to support the light according to applicable codes

(esp. IBC 2000). Check the strength and stability of the mounting structure. It should provide for bracing which will prevent twisting or lateral motion of the light fixture. EQUIPMENT ANCHORAGE DIAGRAMS are supplied with these instructions to help with the installation (Figure 5 & 6). They were prepared by a

California-licensed Structural Engineer, and if the lights are installed accordingly, the systems will meet requirements of the State Seismic Codes.

2.3 Condition of Product

Each BURTON Visionary Light is shipped in two or more cartons. If there is obvious damage, or you suspect hidden damage incurred during shipment, follow the printed instructions in this manual under “UNPACKING”. Otherwise, remove all components for identification and check for completeness of the inventory. Report any missing items to your dealer or the factory at once.

2.4 Location of Components

Burton Medical Products Corp. 4

Included with the installation kit is a junction box containing the transformer, and a doublewide wall switch plate with dimmer control and fuses. The junction box should be mounted above the ceiling, preferably adjacent to the mount because the transformer must be accessed. The wiring run between transformer and lighthead should be kept as short as possible to minimize line losses. The wall plate should be substituted for an existing wall plate at a convenient location, and the wiring dedicated to the Visionary Surgery Light. Each lighthead must have its own dimmer wall plate assembly.

2.5 Installation

Follow the installation instructions in this manual and utilize the Checklist (page

xiv) while assembling the light to make certain that all steps are completed correctly.

2.6 Main Power Connection

In the special case of a Double Ceiling Mounted fixture, the mains power to both wall plates must originate from a single, switched power source so that one breaker switch at the central panel can disconnect power to the entire fixture.

2.7 Transformer Location

The junction box containing the transformer should be located conveniently so the transformer and wiring can be accessed or changed. To minimize line losses between the transformer and fixture, it is recommended that the transformer box be located above a ceiling panel near the fixture mount. Connecting wiring

(contractor-supplied) must be minimum AWG 16 from the transformer to the fixture. Wiring to the transformer must be minimum AWG 18. Wiring and conduit must meet local and national fire protection codes.

WARNING: Both models are intended for installation on a 15-ampere branch circuit.

3.0 MOUNTING AND CHECKOUT

3.1 Head and Arm Movement

The Ceiling Mount and Downtube must be securely mounted to the structural ceiling. Reference the Seismic and plan view drawings (Figures 5 and 6) showing bolt circle dimensions and swing radius of the lighthead(s). Make sure the downtube is located at a point on the ceiling that allows the lighthead to be positioned conveniently above the work area where there will not be interference between any of the moving parts and fixed objects, such as walls, cabinets, other fixtures, etc. The connection from the downtube to the structural ceiling support must be stiff so that there is no ‘sway’ to the loaded downtube, otherwise the lighthead will be difficult to position, and will drift. Also, the downtube itself must be ‘plumb’, i.e., perpendicular to the floor/ceiling. If it is not plumb, the horizontal extension arm always will drift so the lighthead swings lower to the floor.

3.2 Ceiling Mount installation (Single or Double)

3.2.1 Packing list, single ceiling mount: Ceiling plate with spacer tube, extension arm bearing and fixture flange.

3.2.2 Accessories required for installation:

3.4 Maximum Height Adjustment

If the fixture is mounted to a ‘low’ ceiling, e.g., one where the articulating arm can’t be raised to its upper stop, the height of the lighthead above the floor will be limited by the vertical movement of the arm (it hits the ceiling). This situation is illustrated in the Range of Motion Diagrams for the 8’6” (259cm) ceiling example. However, movement of the arm can be adjusted so that it will be prevented from hitting the ceiling (see Figure 12). In the other example, 10’0”

(305cm) ceiling height, the articulating arm can be exercised to its full unrestricted height without hitting the ceiling, and vertical travel doesn’t have to be limited.

The lighthead can be positioned to any convenient height and angle by grasping the handle. It should be easy to do this using one hand. If there seems to be too little or too much resistance to movement, consult Section 5.0 and check the

Friction Adjustments instructions for both the lighthead and the arms.

3.5 Spot Focus

By twisting the handle slightly, the individual spots from each of the three outer high-intensity bulbs can be merged into a single spot in the center of the examination field. During initial setup (power not applied) the handle should be twisted gently to make sure the reflector pods respond properly. This can be verified by looking through the front windows at the reflectors and noting if they rock slightly as the handle is twisted.

3.6 Electrical Checks

Main power to each fixture is controlled by a dimmer mounted to a special doublewide wall switch plate (Fig. 13). This switch plate assembly, manufactured by Burton, contains the on-off switch, dimmer, and two fuses, and should be mounted at a convenient location (e.g., next to the door inside the operating room) where the dimmer can be adjusted by the operator. For a Double Ceiling mounted fixture, two complete switch plate assemblies are required and should be mounted adjacent to each other. They should share a common mains supply, i.e., a single breaker switch will shut off power to both switch plate assemblies at the same

JAMES A HALEY VETERAN'S HOSPITAL

Burton Medical Products Corp. 24 time. Note that wiring to the wall box must contain hot, neutral, and ground leads. Wiring from the switch plate assembly is run (inside conduit) to the transformer junction box, where the appropriate connections are made (see schematic diagrams, Figures 14 and 15). Wiring from the transformer junction box (16AWG or larger) is run to the mount. To test the system after power is connected, make sure the main switch at the wall plate is turned OFF; load both fuses (3-amp) into the holders on the wall plate, and turn the dimmer to its

LOWEST power setting. Now, turn the main switch ON, and check the dimmer

LED indicators. Press the dimmer until the green LED line illuminates, showing that power is applied. The bulbs in the lighthead should be ON, although they might be dim. With full power applied (dimmer at full on), measured voltage at the secondary wires inside the Yoke cover (Figure 16) should not exceed 26.5v

ac. If it does, a different secondary tap from the transformer should be selected.

For inputs between 110 and 134 volts ac, secondary tap combinations of red/gray, red/yellow, or red/orange, should be selected so as to keep the applied bulb voltage between 25 and 26.5 volts at all times (reference Figure 17). For inputs less than 110 v ac, there will be corresponding loss of bulb voltage, and light intensity.

4.0 OPERATION

4.1 Sterilization Protocol – Handle

The Visionary® Handle assembly is extremely rugged and will withstand many, steam autoclave sterilization cycles without deterioration.

Wrap the handle assembly(s) per standard autoclave procedure.

Place the handle assembly(s) in the autoclave.

Set the autoclave cycle for 270°F (132°C), pre-vacuumed cycle.

Set the autoclave cycle time for 3-minutes, turn the sterilizer on, and wait for the process to be completed.

Remove the handle assembly(s) and place it (them) in use, or transport and store it (them) in a sterile environment pending use.

4.2 Applying Power – the Dimmer and On-Off Switches

The main power on-off switch is located on the wall plate. When it is turned on, a green pilot light in the rocker illuminates. The dimmer is next to the power switch. Push the large dimmer switch in and out and see if a line of green LED’s shows that output power from the dimmer to the lighthead is available. If not, there is a very small slider at the bottom of the dimmer used for disconnecting power (the ‘hot’ line only) so minor maintenance, such as bulb changes, can be performed safely. Moving the slider to the left disconnects the dimmer.

Slide it to the right (see Fig. 13). If the LED’s weren’t illuminated before, they should be on now, and all the bulbs in the lighthead should be on. A line of (7)

LED’s to the left of the dimmer lets the user know dimmer status at a glance.

When the dimmer is OFF, a single LED glows dimly to indicate the prior voltage level. When the dimmer is ON, the entire line of LED’s illuminates brightly to show that voltage is applied to the fixture. If some, but not all LED’s are brightly illuminated, this means that output voltage is limited. The setting can be changed by pressing and holding the top or bottom part of the rocker at the right of the switch. Press and hold it at the top end to increase intensity; press and hold it at the bottom end to decrease intensity. The line of green LED’s will provide visual

Burton Medical Products Corp. 32 cues for intensity. When they all are on, light intensity at the work surface should be at maximum. A bulb-saving feature of this dimmer can be observed when first turning the lighthead on. Note that the bulbs gradually, not instantaneously, turn on (‘soft start’). The reverse happens when the fixture is turned off. This single feature may increase bulb life dramatically – on the order of 2x.

Burton recommends that light intensity at the working surface be only as high as necessary to perform the procedure. Extremely high intensities present uncomfortable eye glare, as well as result in reduced bulb life and in thermal stresses on the equipment. When adjusting the dimmer for optimal intensity

(white – high color temperature – non-glare – easy viewing), gradually increase dimmer setting. The dimmer features a memory function, and if the fixture is turned off then restarted, power will automatically ramp up to the previous setting.

4.3 Maneuvering the Lighthead

If a procedure is to be performed which requires a sterile field, the Handle must be sterilized, and at no time during the procedure may any other part of the fixture be touched. The Visionary® arm system and Lighthead attachment were designed so the Lighthead could be positioned at any location within its Range of Motion using only the Handle. To verify this, grasp the Handle with one hand and, gently but firmly, maneuver the Lighthead so that the projected light will illuminate the subject from various angles.

Also, note that a slight twist of the Handle will adjust the beam pattern so that the individual light beams from the three high-power bulbs can be merged at the work surface.

Forces required to manipulate the Lighthead are modest; there should not be

‘hang-ups’ when moving the fixture. Also, once the Lighthead is positioned, it should not move, or ‘drift’ when released. Proper functioning is contingent upon proper installation; if the fixture is not ‘plumb’, it is not secured properly to the ceiling, the set-up adjustments have not been tested, or periodic maintenance has not been performed according to the schedule, maneuverability will be adversely

JAMES A HALEY VETERAN'S HOSPITAL

Burton Medical Products Corp. 33 affected.

After the procedure is completed, the Handle can be removed for cleaning and sterilization by pushing the plunger located between the Handle flange and housing with your thumb. The Handle will slide off its retaining shaft.
