About this file
This is a Past Performance Questionnaire used by the Bureau of Reclamation to evaluate contractor performance on referenced contracts. The questionnaire is part of the solicitation process for the Brackish Groundwater National Desalination Research Facility (BGNDRF) Yard Piping and Flow Measurement Upgrades contract (Solicitation Number 140R4025R0017). Completed questionnaires must be returned by 4:00 p.m. MDT on September 5, 2024, to Contract Specialist Valerie Jiron at vjiron@usbr.gov.
The questionnaire employs a five-tier rating system (Outstanding, Better, Acceptable, Marginal, and Unacceptable) to assess contractor performance across six major categories: Technical Rating (staff qualifications, experience, and data collection/analysis); Management Rating (project management, schedule adherence, scope management, resource allocation, internal communication, and billing accuracy); Quality Rating (product/service quality and problem resolution); and Customer Satisfaction Rating (communication and professional conduct). Respondents must provide detailed explanations for each rating, with particular emphasis required for any adverse ratings (Marginal or Unacceptable), including specific events and reasons why corrective actions were ineffective. The questionnaire also requests general information including contractor strengths, weaknesses, alignment with original proposals, and recommendations for future engagement.
