MATOC SAMPLE Project Repair Laundry PTA 282 & Install Grease Trap PTA 270
Site Plan
PTA 282
DEMOLITION:
1. Remove Power panel, Washer outlet, Dryer outlet, conduit, disconnect switch and all other associated electrical components.
2. Remove plumbing, washer drain, vent, backflow preventer and 4” drain to existing cesspool.
3. Demo wall.
4. Demo Concrete steps
INSTALLATION:
1. Install new wall partition, non structural metal studs, 5/8” moisture resistant drywall, T‐111 T&G wainscot with 1x 2 wood moulding, wood wall base, (all sides of laundry room only) Door Jamb, SC wood door, kaba ilco locks, Door closure with delay action , kick plates, door stopper. T‐11 T&G is for laundry room only.
2. Repair portion of Drywall, 1000 sq ft. Patch wall to match existing, new vinyl wall base. (Whole building)
3. Paint the whole building, inside only; two colors, Wall, door and window trim. Follow existing color.
4. Install Concrete stairs as shown. Includes rebar, dowel, sub base , base course.
5. Install 8 dryer vent hood, with vent flap.
6. Install plumbing plan as shown.
a) 8 washer connection, and hose bib.(with single individual isolation valve)
b) Lint trap See details on page 16
c) New 1 ¼” cut out valve, handhole, 1 ¼” double check valve backflow preventer, 1 ¼ copper line and water meter
d) New 6” sewer line, schedule 40 abs‐pvc
7. Install electrical plan as shown,
a) 400 Amp panel (54 circuits), 3 phase with main 600 Amp SS Circuit breaker disconnect, conduit weatherhead service entrance cap.
b) E METER, SQUARE D PM870, Current transformer, SS Enclosure, hinge and clamp type cover with visual access.
c) 8 ‐ 20 Amp, single phase, 110v, dedicated circuit for each washer.
d) 8 ‐ 30 Amp, single phase, 220v, dedicated circuit for each dryer.
e) 1 ‐ 20 amp circuit; 1 quad receptacle close to entrance door, 2‐quad receptacle at first table, single phase, 110v
f) 1 ‐ 20 amp circuit; 1 quad receptacle close to storage, 2‐quad receptacle at second table, single phase, 110v.
g) Light circuit, 2 control (equal number of lights), including outside lights with photo cell.
h) Separate light control for Laundry room and storage room
8. Install drywell as shown in page 12.
STATEMENT OF WORK‐ LAUNDRY REPAIR
1. Install new 500 gallon grease trap. This is a government furnished equipment located in DPW PTA yard. Approximately 600 meters away in non paved roads. This is a Jensen Precast Grease trap. Install in accordance with the manufacturer’s recommendation www.jensenprecast.com.Follow International association of Plumbing and Mechanical officials (IAPMO)See below for additional information.
2. Install new 4” sewer line, with COTG every 100 ft.
3. Remove and dispose old Grease Trap. Follow Hawaii Department of health guidelines.
NOTES:
a. 1 ½ “ lateral water line in between PTA 280 and PTA 270
b. All excavated material will be disposed in BAAF, see attached map on page 1 and 20.

1. The Statement of Work (SOW) is intended to cover the complete installation of systems to function as described and specified. Omissions and/or errors from the SOW, which are manifestly necessary to carry out the intent of the SOW, or which are customarily performed, shall not relieve the Contractor from performing such omitted or improperly described details of the work and shall be performed as if fully and completely set forth and described in the SOW.
2. Contractor shall verify all conditions and dimensions relating to the project before commencing work. The Contractor shall promptly notify the Contracting Officer’s Representative (COR) of any discrepancies and/or conditions which prevent the contractor from fulfilling the terms of the contract.
3. Contractor shall verify all equipment and hardware dimensions, weights, connection locations, and required clearances for installation and maintenance at the jobsite and coordinate their requirements with the manufacturer prior to procuring material.
4. Prior to commencing work, Contractor shall work with the COR and other users to properly phase project to minimize disruptions to the user. The area may be occupied during the period of this contract. Therefore, all contractor work shall be coordinated with the COR and the user to minimize the inconvenience to the occupants.
5. All work shall comply with all applicable requirements of the current versions of the Unified Facilities Criteria (UFC), International Building Code, Plumbing Code, National Fire Protection Association, NFPA 90A, ASME Pressure Piping Code, Federal OSHA Codes, National Electrical Code, Army Corps of Engineers Safety Handbook (EM385‐1‐1) and all other agencies having jurisdiction.

6. Furnishings that are relocated to allow construction shall be returned and replaced to original locations and condition after completion of construction.
7. All utility outages shall be of as short a duration as possible and shall be scheduled as far in advance as possible with the COR.
A request for any power or water outages shall be submitted to the Contracting Officer (KO) a minimum of 4 weeks prior to the intended outages.
8. At the end of each work day, the Contractor shall remove all debris and garbage from the worksite so as not to allow accumulation inside or outside the building. The entire area shall be cleaned and left in a condition acceptable to the COR.
Materials that cannot be removed daily shall be stored in areas designated by the KO.
9. Existing conditions or areas damaged or disturbed by the contractor's operation shall be restored to their original condition, or near original condition as possible, to the satisfaction of the Contracting Officer at no additional cost to the Government.
This includes, but is not limited to, elements such as structures, utilities, appurtenances, and landscaping.
10. Contractor shall provide GPS coordinates of connection points and bends for newly installed utility lines. Contractor shall provide GPS coordinates for corners of newly installed concrete pads or structures.
11. Contractor shall submit schedule of values within 21 days of contract award for projects at or above $250,000. Schedule of values shall be sent with requested billings to the project inspector and CC the COR to justify the billing requests. Schedule of values shall include price or percent breakdown for task involved in the construction of the project.
12. Demoed Conduit, Power panel, concrete will de accepted by DPW recycling. All others ,will be dump in an approved site.(Hawaii county)
13. Excavated material for new water line, lint trap and new concrete, spread in between PTA 282 and PTA 280 or in BAAF. See attached map.
14. Building will be unoccupied during construction.
15. Coordinate with PTA staff, PTA water provider, DPW Oahu HV shop for power and water outage.
16. No as built available.
17. All installation must comply the latest NEC , plumbing code, building code.
18. E meter equivalent is Square D PM556x with comm capability (if PM870 is not available)
19. Print this SOW in color to identify new, existing.
20. Drawings are not to scale and conceptual only.
Description Note: The KO may request submittals in addition to those specified when deemed necessary to adequately describe the work covered in the SOW.

• If electrical work is required, Activity Hazard Analysis (AHA) must include a copy of the electrician license of the prime electrical contractor
• If scaffolding is used, AHA must include scaffold training certificates for each person working above six feet.

3. If lead and/or asbestos are present in construction materials, AHA must include, as applicable, a lead abatement certification and/or lead awareness training (depends on lead content) for each individual removing the lead containing materials and an abatement certification for each individual removing asbestos containing materials.
4. Contractor shall submit all proposed construction schedules to the KO for review/approval within ten (10) calendar days after issuance of the Notice to Proceed (NTP) unless otherwise specified.
5. If HAZMAT is present, job specific safety plan must include applicable Safety Data Sheets (MSDS).
6. If project requires any road and/or lane closure work that disrupts the installation roadways, a Traffic
Control Plan (TCP) and permit shall be submitted and approved prior to commencement of work. An electronic message board shall be placed as noted in the TCP Permit.
7. If project requires any excavation and/or trenching work, an Excavation Permit shall be submitted and approved prior to commencement of work.
8. It is the Contractor’s responsibility to route TCP Permit and/or Excavation Permit documents. Upon receiving final signature, Contractor shall submit document as a “For Information Only” submittal.
9. Closeout Documents consist of 1 ea – hardcopy and CD of the following, as determined applicable by the COR: Equipment List, Warranty information, O&M Manuals, As‐Built Documents. Contractor shall prepare and train Government personnel in operation and maintenance of modified system/equipment.
10. Contractor shall provide all keys, as applicable, to the KO following final inspection or by project closeout (POP end). Keys shall not be given directly to the user.
11. Contractor shall dispose all recyclable materials in accordance with the DPW Solid Waste Disposal and Diversion Form (at Appendix D). Recycling container can be delivered to the job site upon request.
