About this file
This is a contractor surety limitation form template designed for use in the VISN 7 Construction MATOC solicitation. The document functions as a two-part questionnaire that establishes bonding capacity and financial reliability requirements for offerors competing for construction services under this Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract with the Department of Veterans Affairs Veterans Health Administration.
Section A requires the prospective contractor to provide basic identification information including company name, address, point of contact details, and telephone/email information. Section B, completed by the surety company, requests critical bonding information including the duration of the surety relationship, the contractor's rating, single project bonding limits, aggregate bonding amounts, and historical performance data regarding project completion, insurance refusals, and payment disputes with subcontractors and suppliers. The form explicitly notes that surety responses are source selection sensitive and protected under FAR 15.201, prohibiting disclosure of reference provider names. The completed form, including the guarantee bond, must be submitted with the RFP offer. This attachment serves to verify that offerors possess adequate bonding capacity and financial stability to perform construction, maintenance, alteration, and repair work across eight VISN 7 major medical facilities and related VA-owned properties.
