Sheet1 Attachment 2 - Bid Schedule
ITB 2025-1600-0182/1624-084
The purpose of the cost proposal format below is to allow offerors to submit pricing in a consistent manner that the State can evaluate and score. In accordance with ITB Section 1.02 - Budget, total project cost more than $330,000 will be considered non-responsive.
Instructions for filling in costs: Under "Nightly Services," determine individual cost of Nightly Service #1 and the total to perform that service for 1 month. Multiply that number by 12 to determine the cost of one full year. This will be your total for "Year 1, 1 month x 12" (Cell B4). Do the same for cells C4-K4. Cell L4 contains a formula and will compute the final total for this row. Do the same for Service #2, then #3, and so on.
Under "Twice-Yearly Services, determine the individual cost of Twice-Yearly Service #1, then multiply by 2 to determine the cost for Year 1 (Cell B22). Do the same for cells C22-K22. Cell L22 contains a formula and will compute the final total for this row. Do the same for Services #2 - #4.
Below the table, fill in vendor name and contact information (including "Submitted by").
